EXPERIENCE IN IMPLEMENTATION OF INFORMATION TECHNOLOGIES

IN HEALTHCARE OF CHUVASH REPUBLIC

Abstract. The aim of the work was to analyze the experience of implementation of information technologies in healthcare of the Chuvash Republic. The research materials were the data of official statistics of the Chuvash Republic, a database of the health system of the Chuvash Republic, compiled using the MEDSTAT software, statistical reports of federal state statistical monitoring of the health of the Chuvash Republic. It can be stated that an electronic medical record of the patient, electronic services of making an appointment with the doctor, laboratory diagnostic tests, prescribing and sick leave certificates have been introduced in all medical institutions of the republic. More than 3 thousand telemedicine consultations with the federal medical center were held for residents of the republic, which reduced the time needed to establish a diagnosis and prescribe adequate treatment for patients. Conclusion: The creation of the Regional Scientific Advisory Telemedicine Center made it possible to build a multi-level system of telemedicine consultations – from leading federal clinics to district hospitals and polyclinics in the countryside for all medical care profiles, thereby ensuring both medical and budgetary effectiveness.

Abstract. The purpose of the study was to carry out an analysis of the number of citizens placed on military registration in the Orenburg region over the last decade and to establish the main diseases in young men who influence the formation of a conscription resource for the Armed Forces of the Russian Federation. A significant decrease in recruitment resources has been noted as a result of the «demographic failure». The number of young men placed on military records decreased from 13 337 to 9 560 during the study period. The increase of military-patriotic work in the study region and the improvement of health-improving measures have had a significant impact on the state of health of young men. Suitability for military service increased from 66,1% in 2009 to 78,65% in 2018. Mental and behavioral disorders (26,98%); diseases of the musculoskeletal system and connective tissue (14,82%) and diseases of the eye (10,82%) took the leading positions among the diseases giving the exemption from military service. The effectiveness of health-improving measures appeared in the increased number of treated young men: from 82,25% to 89,48% – conservatively; from 61,48% to 84,65% – treated promptly; from 81,65% to 90,48% – oral sanitization; from 56,41% to 89,3% – tooth prosthesis; from 95,67% to 100% – glasses’ correction. Conclusion: the organization of medical support for the preparation of young people for military service requires interagency cooperation in the work of State and public organizations.

MORBIDITY FEATURES OF NEWBORNS ABANDONED BY PARENTS

Abstract. The purpose is to study the morbidity of newborns – early social orphans for the purpose of detecting the diseases which can provoke the mother’s abandonment of the child. Material of study was source of information: the blank form No 96 «Labour and Delivery Records». The information was being collected from 2009 till 2017 from archives of 9 city hospitals of Chelyabinsk. Results: the level of morbidity of early social orphans, who have been born in the regional center, is (1 357,7±62,1) cases for 1 000 of the corresponding population on the average for the nine-year period of observation. Moreover, this significant index increased (Z<Z05) within this nine-year period of observation: from 921,4 cases for 1 000 of the corresponding population in 2009 to 1520,0 cases in 2017. The rate of growth is 165,0%. The other newborns have got the morbidity rate of (478,1±21,1) cases for 1 000 of the corresponding population for the same period. It has been found out that the first place in the structure of morbidity for early social orphans is taken by the XVI class «Certain conditions, which appear during the perinatal period» with the average specific weight of 83,4% for the period of study. The second place is taken by the XVII class «Inborn anomalies, deformations and chromosomal abnormalities» with the rate of 9,6%. The group «Others» takes 6,2%. The smallest part (0,8%) in the structure of morbidity of early social orphans is the XII class «Diseases of skin and subcutaneous tissue». Conclusion: The level of morbidity of early social orphans exceeds the indices of other newborns by 2,8 times. The critical difference of the structure of morbidity for early social orphans is the XVII class of ICD-10 «Inborn anomalies, deformations and chromosomal abnormalities», which exceed the prevalence of this pathology for other newborns by 3,5 times. Moreover, the Down syndrome occurs 30 times more often than it occurs on the average for newborns in this class of diseases. The Down syndrome is a disease with the high level of risk of social orphan hood.

Abstract. The purpose of the study is to develop preventive measures to improve working conditions and health maintenance of copper-zinc extraction workers at the enterprise for the production of copper, zinc and pyrite concentrates – AO «Uchalinsky Mining and Processing Enterprise». Methods. The hygienic analysis of working conditions included an assessment of dustiness, degree of air pollution of the working area with hazardous substances, levels of industrial noise and vibration, climate parameters, work severity and intensity of workers of the main occupational groups involved in the extraction of copper-zinc ores. Occupational morbidity of the enterprise workers was analyzed between 2008 and 2017 according to the Republican register of occupational diseases. Results. Hygienic studies have established the excess of certain hygienic standards, in particular with respect to physical factors, severity and intensity of work, which may lead to the development of occupational diseases in workers that occur mainly with damage to the peripheral nervous, musculoskeletal, respiratory and hearing organs. Conclusion. The results of the study made it possible to develop and scientifically substantiate a set of measures aimed at improving the conditions and work protection, the state of occupational health of workers involved in the extraction of copper-zinc ores.

AWARENESS OF THE WORKING POPULATION ABOUT THE METHODS

OF PROMOTION OF DISEASE PREVENTION USED IN HEALTH EDUCATION

Abstract. The purpose of the study is to develop recommendations for improving the health education of the working population. Material and methods. The study was conducted in 2018–2019 on the basis of polyclinic 3 of Makhachkala of Dagestan Republic. 466 working people were surveyed using the anonymous survey method. A special questionnaire with 35 questions was developed to collect the information. The work used sociological and statistical methods. Results. The study showed that among the sources of oral propaganda of medical knowledge, the first place is occupied by conversations – 55,4%, the second – health school (19,9%), the third – lectures (15,7%). In the structure of mass propaganda sources, the first place is taken by television – 43,3%, the second – the internet (25,9%), the third – the press (17,2%) and the fourth – the radio (13,6%). It has been established that more than 2/3 of respondents are interested in the prevention of cancer, 71,5% in cardiovascular, 63,4% in neuropsychiatric and 53,2% in endocrine diseases. Among respondents, only 13,8% rated their level of sanitary and hygienic knowledge well, 47,5% satisfactory, and 38,7% unsatisfactory. Conclusions. The first place is occupied by popular science literature – 39,5%, the second – by memos and flyers (18%), the third – by booklets (14,8%) among the sources of print propaganda, according to respondents. The most significant ones in the structure of visual propaganda sources are: television (28,3%), posters (25,9%), slides, drawings – 24,8% and activities to promote a healthy lifestyle – 10,7%.

INCIDENCE ANALYSIS WITH TEMPORARY DISABILITY

IN TATARSTAN REPUBLIC

Abstract. The data of clinical and statistical analysis of incidence with temporary disability in Tatarstan Republic for 10 years were provided. 702 199 total cases of the diseases were registered in 2017 including 62,3% of male and 37,7% female patients. When studying the duration of cases in all nosological groups, except for mental disorders, there was a decrease in the average duration from 3,5 to 29,3%. The incidence analysis, leading to the case of temporary disability, allows to obtain reliable information about the health status of the economically active population, to plan measures for prevention, appropriate comprehensive treatment and rehabilitation of patients.

THE REMOTE EFFICIENCY OF DENTAL CAVITY PREVENTION PROGRAMS

AMONG CHILDREN IN TATARSTAN REPUBLIC

Abstract. The high efficiency of dental decay prevention programs with annual oral cavity sanitation (2015–2018) among children of Tatarstan Republic has been confirmed according to DMFT and SIC indices. 800 school children were examined, including 600 children in key age groups: 6–7; 12; 15 years old and 9 years old (optional). After 5 years, the antidecay effect compared to test studies (2014) in the «Control» group by the DMFT index is 3 times higher (0,73±0,05 and 2,29±1,59) and by the SIC index (1,84) – 1,7 times (3, 09). After 10 years of introducing early prevention programs, the DMFT±SD index within children aged 6–7 years decreased by 17,3%. The minimum value of 1,84 was obtained of SIC index at the age of 12 years with a decrease in the DMFT±SD index by 66,8% (0,73±0,05).

Abstract. Healthcare reforms have been carried out in Moscow since 2010. The basis of the reforms is the observance of the rights of people with mental disorders. The main principles of community-oriented psychiatry were formulated in international documents of WHO in 2005–2013, which regulated the organization and operation of mental health services. The purpose of this study is to analyze and identify the current issues of improving the current regulatory framework in the framework of the organization of medical care for patients with a psychiatric profile, as well as to determine the benefits and problems of the current legislation using the example of the State Budgetary Healthcare Institution «Psychiatric Clinical Hospital No 13 of Healthcare Department of Moscow» (SBHI «PCH No 13 of HDM»). Material and methods. We used the following methods: the study and generalization of experience, sociological, analytical, comparative analysis; the analysis of the current regulatory framework and literature in the framework of the organization of medical care for patients with a psychiatric profile for the study period (2016–2018) in SBHI «PCH No 13 of HDM». The analysis of the current regulatory framework governing the provision of medical, including hospital-replacing care for patients with a psychiatric profile has been carried out. Some advantages of the regulatory framework have been identified (certain security of patients and their relatives in the provision of specialized psychiatric care, a more differentiated approach in the provision of psychiatric care in an involuntary manner, somewhat greater awareness of the activities of the psychiatric service, issues of providing benefits, etc.).Thus, along with the advantages, the problems of the current legislation were noted (inaccuracies in some of the wordings when complying with the requirements of a written consent for hospitalization, lack of a protection service for the rights of patients in the hospital, insufficient protection of medical workers); they required scientific justification for improving the existing regulatory framework in order to improve the provision of psychiatric care.

MEDICAL AND SOCIAL ASSESSMENT OF THE HEALTH INDICATORS OF TEACHING STAFF WORKING IN BIG URBAN CENTERS
(AFTER THE EXAMPLE OF KAZAN CITY)

Abstract. Nowadays pedagogical activity makes high demands to the state of teacher’s health and belongs to the risk group due to great professional, mental and psychological stress. The purpose of the study is a medical and social assessment of the main health indicators of teaching staff in comprehensive schools. The study included 162 teachers of comprehensive schools in Kazan during the 2016/2017 and 2017/2018 academic years. The obtained data were subjected to statistical analyses carried out with the help of variation statistics method and the SPSS STATISTICS 23 software tool. We can conclude in the framework of this study that teachers can be assigned to the risk group for many non-infectious socially significant diseases. However, there are no effective comprehensive personalized preventive measures considering the risks of teaching profession up to these days.

Abstract. The article is dedicated to the activities of the great scientist V.M. Bekhterev who made a valuable contribution to the development of Russian pedagogic and the new science area – pedology. V.M. Bekhterev’s views on a child development, knowledge of children characteristics, personal development and to an integrated approach to the study of the child in a significant period of the first childhood are analyzed. Value of Bekhterev’s activity as one of outstanding followers in the development of progressive ideas of N.I. Pirogov, K.D. Ushinsky and other representatives of the Russian educational and pedologist science is noted. The data fill the gap in the history of Russian pedagogy and can enrich the modern practice of organizing comprehensive assistance to the child in the framework of protecting health and personality development. Theoretical materials can be used to create textbooks for training professional personnel.

Abstract. Purpose of the study is the analysis the impact of various aspects of medical care on patient satisfaction. Material and methods. The analysis was carried out by factor analysis with calculation of correlation coefficients on the basis of patients’ survey data. A survey of doctors on the importance of health care factors for patients and medical staff in the case of treatment of friends or relatives was conducted at the same time. The comparative analysis of the revealed dependences is carried out. Results. The study of patients’ satisfaction with medical care revealed the most significant evaluation factors. The opinion of the medical staff about the importance of the factors of medical care for patients coincides with the opinion of patients, except for the factor of explanation (justification) of the prescribed treatmentConclusion. The results of the sociological survey allowed the further improving the program of lexical analysis of medical documentation and identifying a number of significant defects in the quality of medical care.

Abstract. The purpose of the study was to develop a scientifically based methodology for managing the resources of a medical organization while eliminating violations in the provision of medical care to patients with acute coronary syndrome without raising the ST segment. The developed methodology allows the medical organization to prioritize the use of resources when eliminating violations in accordance with the degree of significance of the risk for the patient and the medical organization itself. The implementation of this methodology in the quality management system of medical care reliably reduces the risk for the patient and medical organization within 24 months after providing medical care in the acute period of the disease by 1,6 and 1,9 times, in the period of chronic forms of coronary heart disease – by 1,4 and 1,7 times, respectively.
