NEW «RUSSIAN DEMOGRAPHIC CROSS»: REASONS AND PROSPECTS

Abctract. The purpose of the work was to note the demographic development. Predicted data about volume of birth rate and death-rate in Russian Federation till 2050, including Tatarstan Republic till 2035, calculation and comparison of crude birth-rate (CBR) and crude death-rate (CDR) are presented. Waves of demographic war’s echo (DVE) are imposed upon the change of birth rate in Russia; the modern fall in level of CBR became the result of the 3-d wave of DVE. The evolution of death-rate of Russians is accompanied by its decrease because of balancing of the age structure of the population. New «demographic cross» (or excess of CDR over CBR) signifies the beginning of the new period of natural decrease, which will be at fall in the birth-rate against the decrease of death-rate that corresponds to demographic transition (DP) in contradiction to 1992–2013. The 3-d wave of DVE is imposed upon the vector of the worldwide demographic development, especially on the 5-th phase of DP in which our country has entered. Probably, the natural decrease of the population will be changed by weakly positive increase in 25 years in Russian Federation, and in 17–19 years in Tatarstan Republic.

THE ROLE OF ALCOHOL IN POPULATION’S MORTALITY
FROM INJURIES AND SOME OTHER EFFECTS OF EXTERNAL CAUSES (after the example Chelyabinsk region)

Abstract. The article analyzes the cases of violent death, including the poisoning with ethyl alcohol. 31 791 acts (all people died from external causes) of the forensic medical examination of the corpse and the conclusions of forensic experts have been studied. Sex and age groups of the risk of mortality from poisoning with ethyl alcohol were determined. The most pronounced tendency of the increase of mortality from poisoning with ethyl alcohol is observed among men in the age group of 35–39 years. Mortality stabilizes in the up-coming age groups with a growth wave in the age groups of 50–54 years, 55–59 years, and reaching a peak age of 60–64 years. Mortality from poisoning by age group among women (in contrast to men) is less; the rare cases are registered in the age group of 25–29 years, and the peak indicator falls on the age groups of 55–59 years and 60–64 years.In order to organize preventive measures and improve the medical care at injuries, poisonings and some other consequences of external factors, it must be taken into account that ethyl alcohol is not only a direct cause of death of the population, but also a leading factor in all violent deaths. More than half of the lethal cases from external causes in the victims revealed alcohol in the blood. The concentration of ethyl alcohol in the blood of the dead shows a growth trend, increasing from 2,31‰ in 2012 to 3,15‰ in 2017. The traditional prevalence of the average content of ethyl alcohol in the blood of dead men has outstripped the female since 2015.

THE EVALUATION OF THE REPRODUCTIVE CAPACITY IN CHUVASH REPUBLIC MUNICIPALITIES

Abstract. The purpose of the study was the development of math model of the reproductive capacity evaluation of municipalities of the region. The math model of the reproductive capacity evaluation is developed using cluster, correlation, regression analyses after the example of Chuvash Republic. The vector-scalar functional is defined and calculated for the first time. It describes the reproductive capacity of the region and municipalities in detail. The results of math model and multi-factor analysis makes possible to see that the birth rate in Chuvashia depends on the proportion of fertile women, abortion rate, fetoinfantile mortality rate, morbidity in children of 1 year old, the mortality rate of working population, the incidence of pregnant women. However, the impact of risk factors on the reproductive capacity differs in the municipalities of Chuvash Republic. It is possible to set any configuration of the vector-scalar capacity and calculate the optimizing criteria of functional, which is the indicator of reproductive capacity of municipality of the region in the created math model.

Abstract. The purpose of the work was the study of functional state of the cardiovascular system in mineral extraction workers. The state of the blood circulation system was examined using Holter heart rate monitoring with the assessment of heart rate variability, daily blood pressure monitoring in 52 oil extraction workers (the first group) and 45 mineral workers (the second group). Ventricular and supra-ventricular extrasystole was recorded at the workers of the first group at Holter heart rate monitoring. Paroxysms of supraventricular tachycardia, atrial fibrillation, and group ventricular extrasystole were found at workers of the second group. There was a decrease in the general heart rate variability. The predominance of the sympathetic nervous system activity was revealed against the background of a low level of vagal influence among mineral extraction workers. Indicators of the daily profile of blood pressure, in particular, the SBP, as well as the level of DBP at night were higher than in the comparison group. The results of the study show that workers with functional disorders of the cardiovascular system should be referred to the risk group of developing diseases of the circulatory system with the implementation of recreational activities.

Abstract. The purpose of the work was to study the morbidity and mortality rates from malignant neoplasms of the working-age population in the Chuvash Republic in 2011–2016. The materials were: official statistical data according to form No 7 «Data on malignant neoplasms» in 2011–2016, data about the distribution of deaths from malignant neoplasms by sex and age (form No 5) of Rosstat and Chuvashstat. The article presents special aspects in morbidity and mortality from malignant neoplasms among working age population in the Chuvash Republic compared with those in the Russian Federation and the Volga Federal District. The analysis of changes in mortality patterns among working age population in 2016 compared to those in 2002 is carried out. The dynamics of detectability of malignant neoplasms during preventive periodic examinations has been presented. Detectability of malignant neoplasms in the breast, in the cervix, in the uterine body, in the ovaries at early stages of tumorous process has increased through the use of integrated prevention and early detection of reproductive system’s tumors in the period from 2002 to 2016 within the framework of a number of federal and regional targeted programs.

THE IMPACT OF LIFE PRIORITIES ON THE SELF-ASSESSMENT OF THE HEALTH OF THE POPULATION OF SENIOR AGE GROUPS

Abstract. The study focuses on the health status of the population of senior age groups. Purpose of the study is the analysis of the impact of life priorities on the self-assessment of the health of the population of senior age groups. The anonymous survey of 967 people was conducted. The mean age of participants of survey was (71,08±1,3) years. The author’s questionnaire suggested to define priorities. The study did not allow us to obtain convincing results about the influence of the hierarchy of life priorities on the self-assessment of health, both in men and women. Health as a vital value is most significant when the level of health is reduced, whereas at a high level it is not considered as a goal, for the attainment of which it is necessary to change behavior.

Аbstract. The purpose of the study was to study the regulatory legal basis governing the use of the per capita financing mechanism and to carry out the analysis of its implementation in different regions of the Russian Federation. The sources of information were: scientific publications and normative legal acts on the problem under study, internal normative legal documentation of polyclinics, «Collection of Moscow average basic indicators of the activities of healthcare institutions of Moscow Healthcare Department» in 2010–2014, «Collection of performance indicators of treatment and prophylactic institutions of Moscow Healthcare Department in the administrative districts» in 2010–2014. The following methods were used: study and generalization of experience, analytical, statistical, etc. In general, the analysis of the implementation of the per capita financing mechanism in the regions of the Russian Federation demonstrates a significant variety of indicators used to assess the effectiveness of healthcare institutions providing primary health care.

MODERN ISSUES OF IMPROVING OF THE SYSTEM OF HEALTH SERVICES’ PROVISION TO THE POPULATION IN THE STATE HEALTHCARE INSTITUTION ON A PAYING BASIS

Аbstract. The purpose of the study was to study some issues of improving health services to the population on paying basis in the state healthcare institution. Sources of information (research materials) were: scientific publications and normative legal acts on the problem under study, internal regulatory legal documentation and the results of sociological survey. The following methods were used: study and generalization of experience, analytical, sociological, etc. The study of the opinions of patients who applied for health services on paying basis in M.V. Vladimirsky Moscow Regional Scientific Research Institute was conducted according to special questionnaire. The article is dedicated to the problems arising during the organization of health services on paying basis in state healthcare institution.

ACMEOLOGICAL CONDITIONS AND FACTORS OF EFFECTIVE TRANSITION TO THE SYSTEM OF CONTINUOUS MEDICAL EDUCATION

Abstract. The purpose of the research was to study the professional opinions of medical workers, to determine the range of issues that require their priority decision during the transition to the system of continuing medical education. We interviewed 86 physicians-retrainees of Biomedical University of Innovation and Continuing Education of Federal State Budgetary Institution «Burnazyan Federal Medical Biophysical Center» of FMBA of Russia (Moscow), employees of AO «Medical Center «Zenit-Medservice» (Krasnogorsk in Moscow Region) and Central Hospital at Gudermes station of OAO «Russian Railways» (Chechen Republic). Content analysis of literary sources on philosophy, psychology, sociology and the organization of healthcare was carried out. First of all, it was noted that the transition to the system of continuing medical education is inevitably accompanied by stress – emotional, mental. The authors consider the trainee as a person with the integration of biological, mental, social process. All this is part of the components of the concept of professional development. Effective transition to the system of continuous medical education requires a number of activities, such as planning and organizing staff training, primarily from the doctors themselves – change of mentality with acmeological positions, professional self-determination together with personal self-determination, its development as a process of forming the inner world in the educational process of continuing medical education.

THE SYSTEM OF MOTIVATIONAL AND VALUE ORIENTATIONS OF INTERNS AND RESIDENTS ACCORDING TO SOCIOLOGICAL SURVEY DATA

Abstract. A sociological survey of the group consisting of 200 people was conducted. The purpose of this study was to identify motivational and value orientations of interns and residents of Far Eastern State Medical University. A group of residents and interns responded to a questionnaire consisting of several blocks, which included standard questions such as gender, age, place of residence (city/countryside), admission and training at Medical University. The questionnaire contained questions about the reason for choosing the University, the completeness of the volume and availability of information received at the University during training, evaluation of its quality. Participants of the sociological survey have answered the questions of the proposed (desired) specialization. The questionnaire addressed the issues of wages, availability of housing, or the prospects of its preferential obtention, working conditions and the state (availability) of the infrastructure of the area (city, village, remote areas of the region). The survey period included 2016–2017. The study involved 60 men and 140 women with an average age of (23,3±1,5) years. According to the results of the study, 81,5% (163) of respondents chose the specialty of physician consciously, 1% (2) considered studying at Medical University prestigious, 8% (16) decided to continue the dynasty, 9,5% (19) of respondents found it difficult to answer why they decided to link their life with medicine. The majority of interns and residents 94,5% (189) plan to continue their work in the health care of the region after graduation. 65,5% (131) plan to work in the outpatient clinic, 39% (78) – in the hospital and 3% (6) want to combine work in the hospital and in the outpatient clinic.

REVISITING THE QUESTION OF THE IMPORTANCE WITH DUE CONSIDERATION FOR OPINIONS OF THE RELATIVES OF POSTHUMOUS DONOR: ANALYZING THE EXPERIENCE OF FOREIGN COUNTRIES AND PUBLIC OPINION
OF RUSSIAN SOCIETY

Abstract. The article examines the problem of the participation of the relatives of posthumous donors in the decision making regarding organ donation. The author has analyzed the legislation of the countries realizing the programs of organ transplantation. It was found that each state carries out the decision regarding the necessity to receive consent of the relatives of posthumous donor taking into consideration the religious or ethical aspects in case of the absence of special directions in international documents. The author has done a survey of three groups of respondents and has made a conclusion about the practicability of extending the will expression of the relatives of posthumous donors in the legal system of our country.

APPLICATION OF SOCIAL MEDIA IN HEALTHCARE

Abstract. This article examines the opportunities of social media integration into the Russian healthcare industry. The authors learned the experience of foreign researchers in this area. The article describes the potential importance of social media for the industry, healthcare institutions and patients, as well as limits of these communication channels. The article analyzes relevant cases from the practice of leading foreign specialists, the advantages and disadvantages of application of social media in the medical field. The authors make the conclusion about the necessary background and opportunities for social media integration into the information infrastructure of Russian Healthcare.

Abstract. The purpose of the study was the creation of possible criterions of content-related integrity of medical records to improve the quality of medical care. Database and software for implementation of lexical and semantic analysis of medical records have been developed. More than five thousand medical records have been analyzed using 30 criterions according to regulatory documents of HM of RF. The audit showed that 87,5% of documents contained the defects of formalized approach upon several criterions. The creation of the interface for primary data processing in medical and non-medical vocabularies allowed making the expert processing of the content. The use of the created system of lexical control will increase the volume of quality control in medical records and improve its quality and efficiency.

Abstract. The aim of this study is the determination of the influence of cross-cultural competence of the healthcare institution’s leader on process of relationship management in the field of healthcare. The modern head of healthcare institution (HI) communicates with superior organizations, colleagues, partners and patients with their relatives, forming the unified professional environment of HI. Recognition of the importance of interlocutor’s personal values and tolerance become indispensable features of communication in such a socially sensitive industry as healthcare. Educational module for healthcare leaders aimed at improving the personal and professional communication skills has been included in the program of professional education of leading healthcare staff at public health and healthcare sub-faculty of A.I. Evdokimov Moscow State University of Medicine and Dentistry of Health Ministry of Russia since 2017. The result of the educational module is the development of practical skills of communication with representatives of different cultures in HI that form the corporate culture of HI and improve the quality and safety of the medical activity in general.

Abstract. This article analyzes the main development trends of researches in healthcare sphere in the European Union (EU). It shows how the innovative approaches and the new technologies (personalized medicine, electronic and mobile healthcare) contribute to high level and quality of life, to well-being of the population and protection from the consequences of demographic changes. It was made a conclusion that the development of medical science, contributing to the improvement of population’s health was the EU reply to global socio-economical pressures. Moreover, the active implementation of advanced technologies and innovative products has extended the boundaries of the modern medicine, increasing effectiveness and modernization of healthcare system.
