Abstract. The work presents data on monitoring of medical and demographic processes — birth rate, mortality of the population of Kazan, including among persons of working age, primary and general morbidity, features of circulatory system diseases, oncological, mental, infectious morbidity, tuberculosis, syphilis, the indicators of the primary establishment of disability in 1995—2016. The peculiarities of general and primary morbidity, chronisaton of diseases, diseases with temporary disability (DTD), socially significant diseases are shown. Information on the forecasting of medical and demographic processes — of general and primary infectious morbidity, socially significant diseases, DTD, disability for the period 2016—2021 is given. Analysis of the prevalence of diseases in all age groups from 1995 to 2016 is given. It increased (by 15%), greater extent among adolescents (by 56,9%) and somewhat less — in children (by 22,5%) and adults (by 21,7%). At the same time, there was tendency to decrease: in children — diseases of the nervous system (by 7,6%) and mental disorders (by 78,8%), in adults — skin diseases, subcutaneous tissue (by 8,7%), nervous system (b 14,2%), mental disorders (by 38,7%), injuries and poisonings by 17,6%. Computations of relative and average values, their errors, determination of the reliability of differences and trends have been carried out.

Abstract. The overweight is the most important problem of the healthcare system and the economy of the industry today. Its indexes have grown more than twice in the world. The article presents the analysis of home and foreign literary sources about the study of overweight among employees of educational institutions. It has also been established that the teacher’s activities had the increased demands both on physical and mental health in connection with its social importance. This study points out to more detailed and in-depth study of the problem of overweight and obesity among academic staff.

MORBIDITY, DISABILITY AND MORTALITY DUE TO CIRCULATORY DISEASES AMONG THE ADULTS OF THE ORENBURG REGION

Abstract. The article presents the levels and the structure of morbidity, disability and mortality due to circulatory diseases among the adult population of the Orenburg region for the period from 2011 till 2016. The study uses statistical and analytical methods. Mortality rates and general disability of the adult population due to circulatory diseases have decreased in 25,1% and in 48,9% for 6 years, while the general incidence rate of the population has increased in 13,4%. The average long-term structure of the general incidence of circulatory diseases was characterized by the presence of diseases with high blood pressure (44%), cerebrovascular diseases (18%), and coronary heart disease (11%). At the same time, coronary heart disease prevailed (53,1%) in the long-term structure of mortality due to circulatory diseases.

Abstract. The mortality due to malignant neoplasms tends to grow and is much higher than the values recommended by the «road map» in the Republic of Tatarstan. The purpose of this study was to study the dynamics and structure of mortality of the adult population of the South-Eastern region of the Republic of Tatarstan from 2006 till 2016. As a result of the study, it was found that mortality due to malignant neoplasms of the population of the South-Eastern region (SER) has increased in 2007—2016 in 22,5%, and was 195,20/0000 in 2016. The rate of mortality growth in the SER is 3,4 times higher than in the whole country.

THE IMPACT OF SOCIAL AND HYGIENIC FACTORS ON THE INCIDENCE OF MYOCARDIAL INFARCTION AND FORECASTING IT AT THE INDIVIDUAL LEVEL

Abstract. Myocardial infarction in terms of morbidity, prevalence, and other criteria is the leading medical and social problem. Prognostication of many social and hygienic risk factors of this pathology remains unexplored. The purpose of the work is to determine the quantitative assessment of the prognostic significance of some social and hygienic risk factors of myocardial infarction. Registration of individual risk factors of myocardial infarction was carried out in 157 patients with this nosology (main group) and 138 healthy individuals (control group). The diagnosis of myocardial infarction was established according to the criteria of the all-Russian Society of Cardiologists. The greatest prognostic value in the development of myocardial infarction has: divorce in the family, a major scandal at work, a serious illness (injury) of a close relative. The use of these risk factors of myocardial infarction in the practical activities of physicians will permit to predict the development of myocardial infarction at patient who addressed for medical care or passed a medical examination.

Abstract. Life and health protection of younger generation of Russia (about 29 million people) is a subject of daily care and attention of the State and its institutes. Specifics of time, the high traumatism and other external reasons of incidence and mortality of children’s population, as well as tragic events which have received a wide public response in children’s holidays houses and other factors impose new, high requirements to this problem, first of all, to optimization of prevention measures. The contingent which is on holidays and rehabilitation (6—7 million persons every year) has a direct bearing on it. Earlier [5, 6] we have told about the condition of modern legal support of health protection of younger generation, including a sanatorium stage and the organization of rest and rehabilitation of children, as well as the issue of security according to Decrees of the President of Russia, Government resolutions and other documents. This article represents the analysis and filing of the documents for the last 2—3 years, which are unknown by heads, experts and the public. They concern many aspects of health and life protection of children during their organized holidays. These documents, as a matter of fact, represent the algorithm of administrative and technological actions with preventive orientation for heads of departments, organizers of children’s holidays, personnel of the relevant institutions and organizations and health workers. They are about the vocational training, the trainings for parents and teenagers. We have applied analytical, statistical, expert methods, as well as the study of experience of territories and organizations.

SEASONAL PREVALENCE OF HOSPITALIZATION IN PRENATAL CENTRE

Abstract. The article considers the probability of seasonal dynamics of hospitalization in the prenatal centre. The study was held on basis of Ganja Prenatal Centre, which is the maternity hospital of the II class for population of Ganja, Naftalan and the regions: Shamkir, Geranboy, Samukh, Gekgel, Gedabek, Dashkesan, Gazakh, Tovuz, Agstafa and Terter. The case of hospitalization of pregnant women in the gestation period of 22 full weeks for obstetric care was the unit of observation. Determination of seasonal prevalence of hospitalization cases was calculated according the following methods: 1) Determination of number of cases per Summer (May, June, July, September, October) and winter (November, December, January, February, March, April) periods; 2) Determination of number of hospitalization cases per seasons of year — winter (from December,21 to March, 20), spring (from March, 21 to June, 20), summer (from June, 21 to September, 20), autumn (from September, 21 to December, 20). The maximum number of hospitalization cases was registered in January (382,3±4,1) and the minimum number in July (177,1±2,0). The difference in number of average month cases of hospitalization is considerable at paired comparison (р>0,05). The number of average month cases of hospitalization is the same in February and March (318,6±3,7 и 322,3±3,6; р>0,05). So, the seasonal prevalence of hospitalization of pregnant women in prenatal centre is objectively conditioned and manifested by a relatively big number of admissions in January-May. The highest and lowest number of cases of hospitalization in prenatal centre was registered in winter and summer, quantity of hospitalization cases in spring is nearly the same in winter, and the same data is nearly similar in summer and autumn. Seasonal prevalence of birth rate, fertility and hospitalization in obstetrical institutions is advisable to evaluate not per months, but per the seasons of the year.

FORECASTING OF PREGNANCY OUTCOMES ACCORDING TO NUCHAL TRANSLUCENCY OF FOETUS IN THE FIRST TRIMESTER

Abstract. The purpose of the study was the assessment of pregnancy outcomes according to nuchal translucency during the first trimester based on documents of urban standards of Sumgait. The unit of study was a woman with a singleton pregnancy, which was under ultrasound screening on 11—13th weeks of gestation period. The total number of women was 1050. Three observation groups were determined according to the size of nuchal translucency in the first trimester: group of women with nuchal translucency <95th percentile (996) on 11—13th weeks of gestation; group of women with nuchal translucency >95th (54) on 11—13th weeks of gestation; group of women with nuchal translucency >97,5 percentile (18) on 11—13th weeks of gestation. The frequency of pregnancy outcomes was assessed according to the above-noted criteria. The differences between the compared groups were estimated by the criterion χ2. The ultrasound screening has showed that nuchal translucency changed within the interval of 0,72—3,89 mm, 0,77—6,45 mm and 0,99—7,89 mm correspondingly on gestation periods 11, 12 and 13 weeks. Differences in the sizes of nuchal translucency of fetuses on 11—13 weeks of gestation age was significant (5—95th percentile) and was 0,81—2,42 mm; 0,86—2,47 mm and 1,07—2,68 mm correspondingly on gestation periods 11, 12 and 13 weeks. Normal size of nuchal translucency according to ultrasound screening on 11, 12 and 13th weeks of gestation age is less than, correspondingly, 2,42; 2,47 and 2,68 mm. When the size of nuchal translucency of the foetus in the first trimester is >95th percentile, the risk of antenatal foetal death, the birth of foetuses with large anomalies, with a small and very small foetal weight increases significantly. The nuchal translucency in the first trimester as a marker for predicting the outcome of pregnancy is characterized by low sensitivity (≤48,2%), high specificity (≥95,1%) and predictive value (≥95,3%).

TRENDS IN SOCIALLY SIGNIFICANT EYES DISEASES’ INCIDENCE IN THE SMALL RURAL DISTRICTS OF THE KRASNODAR TERRITORY

Abstract. Adult cataract, glaucoma, age-related macular degeneration, diabetic retinopathy incidence is the modern social and hygienic problem due to the effects of blindness. The aim of the work was to study the trends of socially significant eye diseases in small rural areas to improve the provision of ophthalmic care. The glaucoma and cataracts incidence was studied among adults in rural areas of the Krasnodar territory in 2012—2016 according to the official statistical data in retrospect. The unfavorable situation caused by a significant rise of glaucoma and cataract incidence was seen in Shcherbinovsky district. The frequency of first identified disease has not changed in Uspensky district. The incidence of glaucoma and cataracts, on the contrary, has representatively decreased in Novopokrovsky district. Mobile teams of ophthalmologists were created to improve ophthalmic care to the population in Scherbinovskiy district.

The desired forms of medical education are determined.

THE VOLUME AND THE NATURE OF OUTPATIENT CARE TO RURAL POPULATION AND WAYS OF ITS OPTIMIZATION

Abstract. The volume and the nature of outpatient care provided by district physicians in 6 rural medical stations according to 27 268 visits in 3 districts of the Republic of Dagestan were studied in order to develop measures to optimize outpatient care for the rural population. The main share of visits to local physicians is represented by patients with respiratory diseases (35,6%), circulatory system (20,6%), musculoskeletal system and connective tissue (9,8%) and patients with digestive diseases (7,5%). The first place in the structure of incidence of outpatient visits is given to polyclinics of Central Regional Hospitals (32,2%), the second — to rural outpatient clinics (30,5%), the third — to midwife stations (29,3%) and the fourth — to consultative polyclinics of Republican institutions — 8%. On average, 30,6% of the rural population do not seek treatment during the year. The population’s appeal to rural outpatient clinics with more than 30% of visits is registered to Primary Health Centers in the structure of incidence of outpatient visits. The frequency of incidence (per 1000 population) of rural residents to the paramedics remains high and is 647,3‰. Methodical recommendations on optimization of out-patient care to the rural population are developed according to the information of the study.

Abstract. The implementation of the organ transplantation program depends on the interaction of specialists from the transplantation center and donor bases, which necessitated the study of factors motivating physicians to participate in the work with transplantologists. The study was conducted according to a questionnaire survey of 266 physicians from donor bases, who were divided into two groups, depending on the activity of participation in the selection of donors. Comparative analysis did not allow obtaining a reliable difference of the majority of the investigated parameters in the isolated groupsof physicians. The administrative resource of the head of the healthcare institution is established as the most significant factor determining the real work of the physicians from the donor bases.
