Abstract. The study determines the importance of interregional differences of medical-demographic indicators for reforming of health care services. The study has been realized in five administrative regions of Azerbaijan (Absheronsky, Shekinsky, Gakhsky, Ismayillinsky, Agdashsky). These regions have been chosen as they were included to the list of pilot regions by Health Ministry of Azerbaijan for healthcare services reforming. Main medical-demographic indicators of regions have been used. Regions can be divided into three groups: big (population ≥160 thousand persons) — Absheronsky and Shekisky, middle (population — 80—160 thousand persons) — Ismayillinsky and Agdashsky, small (population ≤ 80 thousand persons) — Gakhsky. The fertility rate varies in the range of 11,9—21,6‰. The relatively high birth rate is observed in (≥17,9‰) Shekinsky and Ismayillinsky regions, low birth rate is observed in Absheronsky region (≤13,2‰). The mortality rate varies over a wide range (3,8—8,4‰). The low mortality rate is observed in Absheronsky region, a high mortality rate is observed in Gakhsky region. Trends of mortality rate are weakly expressed. More evident interregional differences are due to bed population ratio (7,9—8,0 beds per 10 thousand people in Absheronsky region and 5 times more in Shekinsky region). Interregional differences due to characteristics of demographic processes and health care service resources are clearly seen on the example of pilot regions in realization of reforms program. Positive dynamics of medical-demographic indicators (a high level of natural growth of population) are not associated with adequate changes in resource provision of health services.

Abstract. Injury prevention is multi-component problem therefore the integrated risks assessment of increase in injury rate is necessary in the territory with mass population. Such risks are determined by the external and internal environment of jurisdiction and are called determinants. Identification of determinants and their accounting is important when planning a complex of actions for injury rate prevention.

Abstract. The purpose of the study was the evaluation of morbidity rate of the circulatory system diseases and primary incidence of Tatarstan Republic population in 2005—2014. The analysis and evaluation of data of reporting statistical forms number 30, 47 and operational reporting data of Health Ministry of Tatarstan Republic, State Statistics Committee of Tatarstan Republic, SAHI «Republican Medical Information Analytical Center» have been made. 3910 items of information were analyzed in total. We used the methods of statistical series analysis and linear regression. We found that the indicators of morbidity rate and incidence have sinuous disposition with periods of decline and growth. The results show the moderate decrease trend in primary incidence of the circulatory system diseases in Tatarstan Republic. Predictable indicator has the trend of increase (R2=0,63; р=0,05). At the same time, the average rate of decrease of average quantity in 2011 and 2012 has constituted 33,6%. Negative rate of increase of index of primary incidence for circulatory system diseases is revealed (except for myocardial infarction). The average growth rate is 16,2%.

Abstract. The article represents the study of the trends and formation of infectious case rate in the Republic of Tatarstan in 2005—2015. The analysis was made on the basis of official data of Federal Service for Supervision of Consumer Rights Protection and Human Welfare in the Russian Federation and in the Republic of Tatarstan. The high level of infectious case rate in urban areas and lowest one in rural areas, increased incidence among children have been detected. The most relevant infectious diseases are: acute respiratory infections, HIV-infection, chronic viral hepatitis, acute intestinal infections and HFRS. The results indicate the need of development of the new programs for the prevention and treatment of infectious diseases and optimization of infectious service at the level of the region.

Abstract. The characteristic of health of the children’s and teenage population of the Orenburg region is presented in article. The number of children was enlarged by 7,5%, and teenagers’ number was reduced by 42,8% in the region in 2006—2015. 2015 was characterized by the lowest coefficient of the infantile mortality which made 6,8 on 1000 live births. It is found that indicators of case rate of children of 1 year of life, and also primary and general case rate of children aged from 1 year to 14 years over the analyzed period have been decreased in the region. Decrease of case rate of socially important diseases as tuberculosis and malignant neoplasms was found. At the same time the case rate of syphilis and gonococcal infection among the teenage population was raised. Decrease of disability among the children’s population is registered in the Orenburg region. The alienations and disorders of behavior (36,2%), diseases of nervous system (20,8%) and congenital anomalies (15,4%) prevail in structure of disability of children (aged up to 17 years).

Abstract. Gestational age of 22—27 weeks deserves special attention, because of the Order of Ministry of Health and Social Development of the Russian Federation dated 27.12.2011 № 1687 «About medical criteria of the birth, the form of certificate of birth and the procedure of issuance» from January 1, 2012 pregnancy loss from 22 to 27 completed weeks of gestation in Russia refers to premature birth. The problem of premature birth has the great social importance. Premature birth of sick baby is psychological trauma for the family. This is also the problem of healthcare system, as there is the high cost of the care of premature infants, high frequency of disability, especially among babies with extremely low birth weight.

Abstract. We have analyzed occupational morbidity among mining workers of the Republic of Bashkortostan within the period of 35 years. Diseases caused by physical factors (45,8%), physical overloads and overstraining of certain organs and systems (32,9%) depending on etiological factors rank first in the structure of occupational morbidity among miners. Diseases caused by exposure of industrial aerosols have constituted 13,9%, chemically induced diseases — 7,4%. Almost half of occupational pathologies have been detected among workers of two occupations — excavator operator and drift miner.

Abstract. Cause-effect relations between occupational factors and indicators of the health of workers. The most important factors of production that contributes to the development and progression of chronic non-communicable diseases are the working area air pollution by harmful substances, industrial noise, adverse micro-climatic conditions and the severity of labor. Priority pathology in rubber manufacturing workers are circulatory diseases (52%), diseases of the musculoskeletal system (37.5%), diseases of the ear and mastoid process (16.5%) and diseases of the nervous system (7.6%). The high level of production conditionality for the formation of diseases of the musculoskeletal system (RR-2,3; EF-56%) and the average rate for hypertensive disease (RR = 1,7; EF = 42%) were found. Preventive health recommendations to ensure safe working conditions, health preservation and increase of working ability were worked out on the basis of the study.

Abstract. Structure of public and private interests in the management of national healthcare system is considered in the article. We used the methods: literature review, theoretical analysis method, system analysis method, classification, quantitative and qualitative methods of sociological research. Comparative study of models of functioning of public health services depending on the degree of state intervention has been made. Brief conclusions: the issue of payment for medical services, the presence of private interest in health care has no clear answer and is discussed for a long time. The need of state regulation of the private sector in healthcare and the comparison of its interests with public needs became actual only in the XX century. Diverse international experience shows the important role of the private sector in the functioning and development of health care. Many experts in the field of management point to the fact that the higher the level of state regulation, the less incentive the entrepreneurs to implement their plans.

Abstract. Results of the capacity’s study and nature of outpatient care domiciliary according to 2 435 visits made by primary care physicians are presented. The study was conducted in 2014—2015 on the basis of 4 rural practices of 2 rural districts of the Republic of Dagestan. The method of expert evaluation studied the quality of domiciliary healthcare in 445 cases. The major part of patients (80,1%) with diseases of the respiratory system, circulatory system, musculoskeletal system and diseases of the digestive organs was seen in practice of primary care physicians and general practitioners during outpatient visits. The patients with respiratory diseases, infectious and parasitic diseases (84,0%) were seen in practice of primary care physicians. Medical diagnostic work at general practitioners has constituted 32,2%, at primary care physicians — 37,1%, at pediatricians — 31,8%; and preventive maintenance has constituted 20,6%, 15,2% and 27%. Guideline to improve the accessibility, capacity and quality of home care to the rural population was worked out on the base of the findings.

Abstract. The article reviewed pressing issues of territorial planning of primary healthcare capacity. The analysis was carried out concerning two territorial types differing by proportion of urban population, availability of health care and primary incidence. The highest need in primary healthcare was observed among the population in age group – more than 50 years living in city in the territories with the high primary incidence. These trends don’t depend from gender. Planning of capacity of healthcare is reasonable taking into account territorial features.

Abstract. The article presents legal aspects and problems of the participation of private healthcare institutions in the implementation of regional programs of state guarantees of free health care to citizens at the expense of the system of mandatory health insurance as a form of public-private partnership in the Russian Federation. Participation of private healthcare institutions in the implementation of the provision of free health care program of state guarantees to the population allows use available material and human health resources more efficient, optimize the network of public institutions and increase the accessibility of quality medical services, including specialized ones. Public-private partnership in the health care system is a promising form of investment, the possibility for the population to receive treatment with innovative technologies that yesterday were used exclusively abroad.
