REGIONAL DYNAMICS’ TRENDS OF THE GENERAL MORTALITY LEVEL AND BLOOD CIRCULATORY SYSTEM DISEASES’ MORTALITY IN AZERBAIJAN

Abstract. The article presents the official materials of the State Committee of Statistics of Azerbaijan Republic placed in its web site (www.stat.gov.az). The analysis was made with use of statistical methods of assessment of qualitative signs and dynamics of indicators of health. Comparison of indicators of mortality was carried out from all reasons and from diseases of circulatory system(DCS) throughout the Republic in general, in Baku, Nakhichevan Autonomous Republic (NAR), other regions with differentiation on urban and rural people and the working-age population (for males 15-62 years old, for females 15-57 years old). The main trend was estimated on the 5 years summer intervals (1995, 2000, 2005 and 2010) with addition of data for 2013. It was found that the level of the general mortality and mortality owing to blood circulatory system diseases, share of these diseases among the mortality reasons in Azerbaijan Republic, in its large regional groups (Nakhichevan Autonomous Republic; Baku and other regions) at the population level, among urban and rural people, persons of working-age differs from each other. The mortality is higher in Baku than in Nakhichevan AR. In 1995, 2000, 2005, 2010 and 2013 dynamics of death rate owing to diseases of the blood circulatory system and specific weight of these diseases among the mortality reasons at the population level, among urban and rural people and persons of working-age in the city of Baku, Nakhichevan AR and other regions of the country are described by the regression equations which have reliable approximation (R2≥0,8) and show linear increase of indicators after 2000. Death rate owing to diseases of the blood circulatory system and share of these pathologies among the reasons of working-age persons are more adequate for detection of regional features and the main trends of mortality’s risk because of unequal age structure of population. The risk of mortality owing to blood circulatory system diseases is high in Baku in comparison with Nakhichevan AR and other regions of the country. The community of trend of mortality dynamics of the population in regions and among various groups of population allows coordinate preventive measures against risk factors of cardiovascular pathologies.

SOCIAL AND HYGIENIC MONITORING OF HEALTH OF CITIZENS KAZAN DURING THE SOCIO-ECONOMIC REFORMS OF 1995—2012

Abstract. The article presents the data of monitoring of medical and demographic processes —birth rate and mortality of population of the city of Kazan among people of working age, primary and general morbidity, characteristics of diseases of the circulatory system, cancer, mental, infectious diseases, tuberculosis, syphilis and primary disablement diagnosis in 1995—2012. The character of the general and primary morbidity of chronic diseases with temporary disability(TD), social dependent diseases are shown. Data of prognosis of general and primary infectious diseases, social dependent diseases with TD and disability in 2012 — 2016 are presented. The analysis of the morbidity rate is presented from 1995 till 2012 in all age groups (with increasing on 21,7%), in adolescents (on 51,1%), in children (on 34,9%) and in adults (on 21,7%). At the same time there was the reducing trend: in children — diseases of the nervous system (7,6%) and mental disorders (78,8%), in adults — diseases of the skin, subcutaneous tissue (8,7%), nervous system (14,2%), mental disorders (38,7%), injuries and poisoning by 17,6%. The calculation of relative and average values, their errors, significance of differences and identification of trends has been made.

GENDER CHARACTERISTIC OF PREVALENCE OF GLAUCOMA AND ITS RISK FACTORS

Abstract. The purpose of the study was the assessment of the prevalence of glaucoma and its risk factors among men and women and detection of its gender characteristics. The study was conducted in Ganja with 329 thousand of population, where the demographic aging rate was high. The citizens of 20 years old and elder were the object of the study. The sample size to obtain representative results (6585) was justified taking into account the probability of glaucoma prevalence. Examination was conducted by the ophthalmologist during 2015, using the diagnostic methods according to the clinical protocols. The prevalence of glaucoma among men of 20 years old and elder consists 2,73±0,28% (95% is 2,17—3,29%) and was significantly less than prevalence of glaucoma among women (3,95±0,34%); 95% — confidence interval; (3,27—4,63%). Prevalence of glaucoma among men and women of 20—29 years old (0,64±0,25 and 1,23±0,36%; p?0,05), 30—39 years old (0,81±0,33 and 1,71±0,47%; p?0,05), 40—49 years old (2,79±0,63 and 3,74±0,73%; p?0,05), 50—59 years old (4,62±0,90 and 5,62±0,96%; p?0,05), 60—69 years old (10,18±2,06 and14,15±2,38%; p?0,05),70 years old and elder (6,90±1,92 and 9,68±2,17%; p?0,05) is different, but reliability of difference is not confirmed. Prevalence of glaucoma among men is significantly higher than among women elder than 20 years old (2,73±0,28 и 3,95± 0,34%). Gender difference at prevalence of glaucoma among population of the same age intervals is not very significant statistically (p?0,05). Average risk rate of prevalence of glaucoma among women in comparison with men is similar on the population level (1,28—1,60) on basis of risk factors and without them. Increasing of glaucoma risk for women and men on basis of risk factors is similar and consists 19,4—21,8 for high pressure in the eye ?21 мм Hg, 42,4—49,8 for family history, 5,6—5,7 for diabetes, 3,6—3,8 for myopia, 2,3—2,7 for cataract, 1,7—2,7 for arterial hypertension, 6,1—6,4 for gastric ulcer and duodenal ulcer.

DISCRIMINATIVE ANALYSIS AS PROGNOSIS METHOD OF RISK OF INFANTILE DEATH RATE

Abstract. The decline of index of infantile death rate is marked on 34,5% in Omsk area in the period of 2004—2013. Discriminative analysis was made in order to prognosticate the risk of infantile death rate. A sample size for the investigated and control group has constituted 225 examinations. Risk factors data of factor analysis are used as the results of our study. Evaluation tables in order to divide all inspected on groups depending on the degree of probability of death of child on the first year of life are made for practical application of results of discriminative analysis. Application of this methodology of estimation of probability of infantile death rate on the basis of prognosis tables will allow the pediatricians to detect risk group in time and to plan necessary medical and preventive care.

PERINATAL LOSSES DEPENDING ON GESTATION TERMS IN THE REGIONS WITH DIFFERENT CAPACITY OF OBSTETRICAL SERVICE (after the example of Barda and Beylagan regions of Azerbaijan Republic)

Abstract. Comparative assessment of prenatal losses risk depending on gestation terms at the time of pregnancy completion has been made in the regions with the different level of capacity of obstetrical service (after the example of Barda and Beylagan regions of Azerbaijan Republic). A sampling unit in our work was the case of completion of pregnancy at the gestation terms of 22 completed weeks and more. Observation was made by continuous coverage of all cases in 2012—2013 (7351 cases in Barda region, 3702 cases in Beylagan region). Complete cases of pregnancy were distributed on 3 groups on gestation terms (22-27, 28-36, 37 completed weeks and more). Fetuses not depending on viability were grouped by body weight at the moment of birth: 500-999 g, 1000-1499 g, 1500-1999 g, 2000-2499 g, 2500 g and more. Frequency of stillbirth rate and prenatal mortality rate was defined according to criteria of WHO. According to the study, the existing practice of distribution of pregnant women on stages of the obstetrical care taking into account the antenatal risks has impact on structures of complete pregnancies on gestation terms in the regional stage of obstetrical service. The probability of stillbirth and prenatal mortality at old (after 28 weeks) and new (after 22 weeks) versions of the account depends on structure of completed pregnancies on gestation terms. The corresponding coefficients will increase at new version, the size of increase will change in connection with change of complete cases structure of pregnancy on gestation terms.

ECONOMIC DAMAGE FROM LOSS OF HEALTH DUE TO CARDIOVASCULAR DISEASES (after the example of Omsk Region)

Abstract. The algorithm for estimation of the economic damage from loss of health of the working population after the example of one of the regions of Russia — Omsk region is presented in the study. This algorithm is universal and allows to justify the priority areas of health care financing in the region with limited funding. It is shown that the structure of the economic damage due to any group of diseases in the population on test can be represented as the sum of the absolute and relative economic damage. It was found that the economic damage from loss of health due to circulatory diseases is considerable for the economy of the analyzed region. The study points to the need for assessment of the adequacy of resource support of measures to reduce morbidity, disability and mortality due to cardiovascular diseases in the region.

CLINICAL AND ECONOMIC ANALYSIS OF EFFECTIVENESS OF DIFFERENT SURGICAL TACTICS IN PATIENTS WITH ACUTE PANCREATITIS

Abstract. The article presents the results of clinical and economic analysis of different surgical tactics in patients with acute pancreatitis. The traditional method of treatment using laparotomy, lumbotomy was used in the control group at 108 patients. Non-invasive methods for the treatment of acute pancreatitis were used in the study group at 98 patients. The degree of relationship between variables was assessed with using of non-parametric Spearman correlation coefficient. All values are represented as M (Medium importance) ± δ (standard deviation) or the frequency (%). Differences were considered statistically significant at p<0,05. We determined the complications, mortality in the main and in the control groups. This approach allowed us to reduce the overall postoperative mortality rate to 13.4%. Active-expectant management is cost-effective in infected pancreatic necrosis. It amounted to save 52000 rubles. Active-watchful waiting is recommended for specialized surgical hospital patients with acute destructive pancreatitis and, in particular, with the infected pancreatic necrosis.

DEVELOPMENT OF THE COMPUTER MODULE FOR OPTIMIZATION OF MEDICAMENTS’ PURCHASING WITHIN TARGET PROGRAMS IN THE REPUBLIC OF TATARSTAN

Abstract. Sociological survey of the experts of the Volga Federal District engaged in government medicaments’ purchasing has been made for the purpose of development of the computer module for optimization of medicaments’ purchasing within target programs. More than half of all respondents have noted that application of information and computer products will reduce labor input and period of operation at the preparatory stage for implementation of medicaments’ purchasing. The specification for creation of the computer product allowing to put into format the information on medicines necessary for the analysis of the prices has been created. Application of the computer module developed on the basis of Microsoft Excel 2007 will reduce labor costs in the analysis of the prices of medicines and, therefore, will allow to optimize the preparatory activities which are carried out within the framework of government medicaments’ purchasing.

Abstract. The article reflects the results of study dedicated to the influence of various factors on changes of epidemiologic situation concerning cardiovascular diseases in big region of the Russian Federation. Multifactor analysis methods allowed to discover that the significant decrease of cardiovascular incidence rate as well as mortality rate in some municipal districts was mainly achieved by great resource potential of local health care systems, and in particular existence of health centers in their structure.

Abstract. The excess of epidemic level of influenza and acute respiratory diseases in St. Petersburg in January 2016 caused the reprofiling of multifunctional hospital SPb SBHI «St. George Municipal Hospital» into infectious hospital. Treatment of 906 patients with acute respiratory diseases has been realized in January-February 2016. 98 patients were hospitalized in grave or extremely grave conditions and needed treatment in intensive care unit. The hospital reprofiling caused difficulties related to treatment and diagnosing process and to abiding to sanitary and epidemiological requirements. These difficulties entailed mobilization of full hospital resources, additional equipment, purchase of secondary medicines.

Abstract. The article presents the study of 119 patients of 19—28 years old in order to detect the medical, social and personal special aspects of drug addiction in status of prolonged remission in the conditions of rehabilitation center. The study was based on sociological (anonymous survey) and psycho-diagnostic methods. The medical, social and psychological characteristics of person’s life, the education in the childhood have been examined. The factors into addictive behavior have been detected. The approach stated above revealed a number of factors which contribute to the development of addictive behavior. The survey has shown that the popularity of classic drugs came down but the addiction from new synthetic drugs has become more widespread. The majority of respondents had the experience of use of various types of drugs. The results of the psycho-diagnostic testing show that the individuals who suffer from depression and exhibit schizoid and hypomanic behavior are more susceptible to the addiction’s developing. Further examination of this group of people may reveal factors that can positively affect person’s motivation to join the rehabilitation program and, therefore, increase the effectiveness of the rehabilitation.
