Assessment of physical development and lifestyle of students at the initial grade

Abstract. The individual health depends on life style in many aspects. The physical development is the most important criterion of health status. Objective of this research was to estimate physical development and life style of first-year students. We used the following methods: sociological (questioning), physiometric, somatoscopic, functional tests, statistical, correlation. The most students showed weak and very weak body build, unfavorable reaction of the cardiovascular system to the anoxia. Considerable violations in life style of students are established: the irrational regime of work and rest, the improper food, the insufficient physical activity, the incomplete dream, and the inattention to cold training.

Abstract. The goal of the study was to analyze the health and social indicators among the citizens, conscripts and male citizens who have expressed voluntary desire to enter military service under contract in the Armed Forces of the Russian Federation. Results of the study showed that the best results were found among the future contract soldiers. The advantages of this category of people are: the average age is between 21 and 26 years old, 11,9% have higher education, 7,06% have incomplete secondary education. 29,35% of the future contract soldiers consume alcohol 1 time a month and more (or 44,53% of all conscripts). The level of nervous and mental stability and cognitive abilities are significantly lower among the conscript young people. 78,6% of the population hadn’t permanent job, and 13,88% hadn’t own separate housing, 31,53% were married and 27,53% had children for the moment of entering the contact service. The results of this study may be used during the mixed brining up to strength of Armed Forces by conscripts and citizens, who have expressed voluntary desire to enter military service under the contract, in order to proper and expedient distribution of incoming recruitment to various military specialties in the overland units and the Navy.

Morbidity rate assessment of prison population according to detailed medical examination

Abstract. Our image about morbidity rate of prison population has been extended on the base of detailed medical examination’s results. 708 persons working in the punishment’s criminal system institutions of Tatarstan Republic in 2014 were examined. Frequency of main pathology depending on the age, length of service and specification of concrete enterprise has been detected. New diseases have been detected in 45,6% of persons for the first time by the results of medical examination. 2,4% of persons have been directed to stationary treatment, 4,1% have received recommendations on the working conditions according their health status, 6,2% of persons have been placed on diet therapy.

Level and structure of disability on profiles of medical specialties of out-patient and polyclinic institutions

Abstract. The data of comparative analysis of level and structure of disability on profiles of the medical specialties in the sphere of the medical and rehabilitation help before disability categorization have been presented. The patient examined in medical-social experts’ commission with confirmation of permanent disability (disabled person) was chosen as an observation statistical unit. The object of observation included all disabled people of the large administrative region (Absheron region with the population of 200 thousand people) examined within full calendar year (2012). The population size has constituted 3010 observation units. Statistical processing was carried out by methods of the analysis of qualitative signs. Comparison of ranks of profiles of medical specialties on frequency of primary disability (neurology, cardiology, general therapy and surgery) and secondary disability (neurology, surgery, cardiology and psychiatry) in male population shows that the neurology rank (the first place) is stable, the rank of cardiology is high in primary disability, but surgery and psychiatry in secondary disability. It indicates the different degree of cumulation of disabled people which can change with death or with rehabilitation of disabled people. A ratio of secondary and primary disability (criterion of cumulativeness of disabled people) in male population is rather high on profiles of psychiatry (8,0) and surgery (5,2), and is rather low on profiles of endocrinology (1,0) and oncology (1,5). The analysis of level and structure of disability on profiles of attending physicians shows degree of time sheet of these experts in connection with treatment of chronic patients. Primary disability of the population is formed mainly by pathologies of neurologic and cardiologic profiles in male, all-therapeutic and oncological profiles in female population. Secondary disability of the population is formed mainly by pathologies of neurologic, surgical, cardiologic and psychiatric profiles in male, surgical and neurologic profiles in female population. Essential distinction of levels of disability depending on profile of profession of the attending physician can generate need of revision of regular standards of physicians for out-patient and polyclinic institutions because of formation of prerequisites of their time sheet during treatment of chronic patients.

Medical-social assessment of the reasons and risk factors of infantile mortality progression on the example of Omsk region

Abstract The decrease of infantile mortality on 34,5% is registered in Omsk region in the period of 2004—2013. The factorial analysis has done in order to study medical-social risk factors of infantile mortality progression. Selection volume for the studied and control group has constituted 225 monitoring cases. The factorial analysis allowed to reveal eight medical-social risk factors of infantile mortality progression. Neonatal factor (22,0%) is on the first place, medical factors (12,5%) are on the second place, obstetric factor (10,8%) is given to the third place of the total community of factors. The study of the signs of different factors has allowed to develop the organizational actions directed to the decrease of infantile mortality.

Abstract. Preeclampsia is one of the most dangerous complications of gestation course. Preeclampsia takes the first place of maternal mortality according to WHO’s data, causing the premature labor, premature placental abruption, placental dysfunction, fetal growth retardation, delivery of underweight children. The number of pregnant women with extragenital pathologies and chronic inflammatory diseases of the pelvic organs, that cause complications during pregnancy, increases every year. Analytic assessment of the gestation course, delivery, postpartum period and newborn health quality in patients with preeclampsia has been done. Mistakes during prenatal care and the lack of preconception preparation in the patients of high risk group, which causes grate number of emergency surgery for health reasons, adverse perinatal outcomes have been revealed. These facts need to optimize preventive measures in outpatients’ services of maternal and child health protection.

Indicators of 24-hour blood pressure monitoring in pregnant women with chronic and gestational hypertension

Abstract. The object of the research was the 24-hour blood pressure (BP) profile study in pregnant women with hypertension. 117 pregnant women with elevated blood pressure without proteinuria, from 18 to 40 years old were examined. Gestational hypertension (I group) was diagnosed in 95 women (81,2%) and chronic hypertension (II group) was diagnosed in 22 women (18,8%). The average age of pregnant women was (30,6±6.4) years old in group I and (28,9±4,7) years old in group II. Gestational age was from 8 to 16 weeks. The average systolic blood pressure (SBP) and diastolic blood pressure(DBP) in the night, daytime and for 24 hours; daily index (DI); time index (TI) of blood pressure; degree of night fall (DNF); average pulse pressure (APP) per night; average dynamic pressure per night (ADP) have been determined. The study showed that there are no significant differences in clinical and anamnestic indicators and in the general and biochemical blood analysis. Significant changes in the daily SBP, DBP and TI SBP in pregnant women with chronic hypertension, as well as the information content of the indicators such as average systolic and diastolic blood pressure, as well as TI SBP, TI DBP in different intervals have been noted.

Abstract. This reseach is dedicated to the study of behavioral and motivational aspects of the patients in oncological alertness and oncological diseases key figures development determined at the dentist. The risk group was determined (more than 4%) using the special methodology of questionnaire. Lack of impulse of the patients was determined as the condition of minimizing cancer causing factors such as tobacco smoking — 24,3% and consumption of alcohol — 16,7%. Implementation of additional dental examination with the use of chemiluminescent method as part of the internal programs of the office targeted on early cancer detection can raise the effectiveness of early detection up to 83%.

Abstract. One of the areas of quantitative interpretation of causal relationships in the system «environment — human health» is the application of methods of health risk assessment. There are two principal directions in research of health risk assessment: assessment of the potential (predicted) risk and assessment of the real (epidemiological) risk. The purpose of this study was to evaluate the epidemiological risk of kidney cancer and bladder cancer under the influence of environmental factors in different bioclimatic and ecological zones of Primorsky territory. The risk assessment was conducted according to A.I. Potapov method (Epidemiological Assessment of health risks at the population level health rankings territories). The input data to calculate the background level were standardized ratio (using the direct method) of kidney and bladder cancer morbidity in the administrative territories of the Primorsky region for 20 years (from 1994 till 2013). The ranking according to the method of Sigma assessment allowed us to identify groups of areas with low, medium and high morbidity of malignant neoplasms of the bladder and kidneys. In these groups represented cities and regions of all climate and geographical areas of the region with different levels of intensity and structure of industrial and agricultural activities. The high prevalence of urologic pathology in the areas of critical and stressful environmental conditions, where the coal, mining and chemical industry, construction industry, mechanical engineering and in areas with intensive use of chemicals in agriculture. The increase of the morbidity of malignant neoplasms of the bladder and kidney is due to the deterioration of environmental quality in connection with the increase of anthropogenic pressures. It was revealed that bladder cancer had a tendency to rise from continental bioclimatic zones to the coast in all ecological zones, which is largely due to differences in the structure of the bioclimate of the coast and continental areas of the region. Calculation of epidemiological risk indicators and ranking of areas according to the risk degree gives the ability to make operational decisions on the development of priority for specific territories targeted preventive activities and health orientation.

Factors determining the human resources of primary healthcare

Abstract. Medical specialists of out-patient care department are the most in-demand personnel in Russian healthcare. The purpose of the research was the study of students’ opinion about professional activity of primary care physicians with revealing the factors influencing to the choice of medical profession. 580 students of fifth and sixth year of SBEI HPE «Orenburg State Medical University» of Health Ministry of Russia took part in the survey. Factors influencing to the medical profession choice are: age, family status, employment, obtained secondary medical education and grade point average of the student. The priorities of medical profession’s choice are: the occupational prestige and the salary.

Nurse staffing of regional healthcare system

Abstract. Analysis of nursing staff resources in healthcare system of Sverdlovsk and Kurgan regions has been made. Provision of population with nursing staff in both regions was stable, but intermunicipal differences reached 5,5 and 2,5 times, correspondingly in 2011—2014. Reducing of specialists’ number of the three main specialties — «general medicine», «obstetrics», «surgery», was noted. The main source of nursing personnel recruitment is the admission of graduates of medical colleges, and main leaving reason is retirement. It is necessary to increase the number of students of the medical colleges and to implement the programs for legal confirmation the nursing staff at working places in municipal and state medical institutions for improving the staff situation in the region.

The results of the activities of Affiliated center in Tatarstan of the North Cochrane Center, created in Kazan Federal University on the base of basic and clinical pharmacology sub-faculty are presented in brief. Russian project of translations of summaries of Cochrane systematic reviews is presented as the basis of the communication strategy for delivering the results of independent Cochrane systematic reviews in plain language to the public.

Health protection of the population: relevant sectors and determinants

Abstract. The study explains the possibility of practically influence of the sectors engaged in public health protection on the factors producing the effect on this process. The purpose of the research was the study of the significance of the engaged sectors’ influence on health determinants. Anonymous questioning of 277 physicians, healthcare managers was the method of the research. 23 sectors engaged in public health protection, their significance, proportion of activities and influences on each health determinant were established. The obtained data may be used for purposeful formation of special programs aimed to protect health of the population.

Assessment of social and medical-prophylactic activity’s level of primary physicians

Abstract. The main purpose of interdepartmental interaction of healthcare and education institutions, regional providers of work is the level increase of medical informedness of population and prophylactic activity. We have studied the subjective assessment of level of professional education in prophylactic work among primary physicians. Assessment’s results can be used in development of organizational-methodical work of primary physicians and in planning and realization of medical-social events among physicians of out-patient department.

Correspondence assessment of the program standard of laboratory analyses at sanatorium treatment

Abstract. The purpose of the study was to estimate the correlation of treatment outcome with implementation of the laboratory analyses programs included in standards of the sanatorium help to patients with various pathologies. Researches were carried out on the basis of sanatorium institutions of JSC «Kurort» (neurologic, cardiologic, gastroenterological, urological, arthrologic mono profile sanatoria, multi-type sanatorium «Naftalan»). Comparisons of the actual frequency of laboratory analyses with their frequency of the sanatorium help provided by standards according to the order of Health Ministry of the Russian Federation were carried out. The real frequencies of prescriptions by all types of laboratory analyses significantly exceed frequencies of the prescriptions provided by standards. The ratio of the real and standard frequencies of prescriptions fluctuates in wide limits (1:1 to 5:1). The obtained data show that statistical significant difference (χ2≥3,8; υ = 1,0; р<0,05) comes to the frequency of prescription of the analysis of glucose, triglycerides, lipoproteins, phospholipids, lactatedehydrogenase, creatinekinase and prothrombin time in blood between subgroups of patients with positive and negative outcome of treatment of the circulatory diseases (CD). This difference is significantly higher in subgroup with negative outcome of treatment. As the conclusion we can say that generally sanatorium institutions carry out the program of clinical-laboratory monitoring of recreants’ status; requirement of additional laboratory analyses significantly increases during sanatorium treatment in connection with various circumstances (deterioration in the patient’s condition, associated disease recurrence, personal desire of the patient and other); it is unreasonable to use the sanatorium standards for restriction of clinical-laboratory examination programs of recreant.
