KSMA — 95th a nniversa ry: continuous development course, scientific research a nd results

Abstract. Kazan State Medical Academy traces its history from the distant 1920s of the last century. All 95 years of its existence are the years of establishment and development, hard work and searches. Nowadays Kazan State Medical Academy rightly has a reputation as one of the most prestigious schools in the country implementing unique educational programs and innovations meeting the practical needs of national health care. Academy also keeps the traditions founded 95 years ago. The article presents data of the Academy achievement and its development prospects.

Medical-social portrait of military doing call-up military service in Оrenburg region

Abstract. The article gives results of issue, concerning medico-social features of military doing call-up military service in Orenburg region. The social portrait of respondents has the following characteristics: average age – 19,8 years old, secondary education & permanent job, growing in full family, single, religious, good income. Besides, there is a high level of bad habits (smoking — 56,0%, alcohol — 36,6%, beer — 41,9%, drugs — 2,5%).

Abstract. Individual health indicators are decreased in mature children born with fetal growth retardation syndrome (FGRS). They have high morbidity and disability rates, wich must be considered as the medical and social problem. The sociological survey with interviewing has been conducted in examined women in order to study the factors influencing the development of the fetal growth retardation syndrome. The main reason of children’s birth with low birth weight was fetal hypoxia due to chronic placental insufficiency in 75,0% of women with asymmetric FGRS and in 100% of women with symmetric FGRS. Pathology of the uterus with personal history according to US data (chronic endometritis in 37,5%, and 88,0% of patients, adenomyosis in 37,5% and 88,0% of patients) is the biomedical risk factor influencing the formation of functionally defective placenta. It requires optimization of organizational measures for preconception preparation of women with this pathology.

Medical-social prevention of algodismenorrhea as main form of menstrual function disorder in teen-age girls

Abstract. 1 134 girls — students of 15—18 years old having no sexual relations from Bishkek general education institutions took part in the study. A test group (402 persons) was composed of the girls with menstrual function disorders in the form of algodismenorrhea and a control one (732 persons) was formed of gynecological healthy girls. A questionnaire survey of the girls-students’ mothers was performed simultaneously. A comparative analysis of medical-social factors of differences’ accuracy among the girls of control and test groups was carried out. Then a prediction risk matrix of menstrual function disorders formation was developed. The use of this matrix in the outpatient attendance will allow not only to identify individual risk, but to take it as a basis for medical-social prevention of reproduction health disorders.

Risk management opportunities in efficiency improvement of reperfusion therapy in myocardial infarction

Abstract. The data analysis of literature about reperfusion therapy in myocardial infarction (MI) has been given. It has been found that thrombolytic therapy (TT) and angioplasty of coronary arteries were the most effective MI treatment methods. Problem questions of reperfusion therapy have been shown. The main adaptation directions of risk management methodology to the medical practice have been presented on the base of literature data analysis. Using opportunities of risk management technology have been marked; the need of its implementation for efficiency improvement of reperfusion therapy and angioplasty of coronary arteries in myocardial infarction has been proved. The risks and situation risks of MI appearing in diagnostic and treatment during reperfusion therapy have been reviewed.

Multidimensional assessment and priorities of disability prevention due to ophthalmic pathology

Abstract. The aim of the study was to justify a complex criterion for multi-dimensional assessment of disability due to ophthalmic pathology. We used the following criteria to calculate this criterion: the number of invalids of I, II and III group (risk and severity of disability); the patient’s age at the time of initial recognizing him/her disabled (the characteristic of loss of employment potential); the average life expectancy of disabled person (life expectancy associated with disability). The multidimensional characteristic of disability is expressed in single criterion — a complex assessment of medical and social burden (CAMSB) of disability, calculated using 3 factors: number of the disabled persons differentiated by groups; disability groups’ points; average life expectancy of the disabled person. It has been found out that the place and role of certain eye diseases in the formation of the disability burden varied depending on the used evaluation criteria. The leading diseases by the risk of disability of the population are: diseases of crystalline lens (1st place), retina (2nd place), myopia (3rd place), eye injury (4th place), glaucoma (5th place), and optic nerve (6th place). The leading disease by the risk of disability of I group (blindness) is glaucoma (1st place), and by the risk of early development of disability (the patient’s age initially recognized as disabled) are refraction and accommodation disturbances. The place and role of eye diseases in the disability of the population have been determined according to the role of all criteria by CAMSB. Myopia, diseases of lens, retina, optic nerve and eye injuries are the leading pathologies.

Personal potential evaluation of MPI healthcare personnel using information technology

Abstract. The estimate results of the regional budget expenditures over three years' period on minor medicines, carried out with the help of ABC/VEN-analysis, are presented. The results show that multi-type healthcare institution uses poorly studied expensive drugs with unproven efficacy, which is irrational. This situation is largely due to the existing regulatory framework. The approaches to improve medicine use should be comprehensive and include bringing current standards and recommendations in accordance with the existing evidence-based information on the true efficacy of medicines.

Abstract. The article analyzes the legal framework sanatorium activity at the present stage of development and relevant organizational and management approaches to the implementation of spa services are proposed. The traditional classical analysis of legislative documents is directed on integration of the data of rather organizational and legal bases of sanatorium activity. Object of study became federal legislative and bylaws in dynamics for the last 20 years. The need in development of the new concept considering realities of the present, allowing not only to restore in certain degree the conventional scientific and practical achievements lost during reforming in sanatorium business but also to enlarge the sphere of sanatorium treatment at the present stage of development of sanatorium business. Implementation of the new concept will help to improve the infrastructure conditions of resorts and therapeutic areas, ensuring economic stability and profitability of all health resort, will allow to keep and use rationally the most valuable natural medical resources of Russia, to increase the level of rendering sanatorium services and competitiveness of domestic resort complex in the international market. In general, services have to ensure safety, health and life protection of the population, environmental protection, complexity, accuracy and timeliness of execution, and also recommendatory requirements to types of service (appointments, comforts, esthetic, etc.), including service conditions. Thus, the modern state policy in resort business has to be based on priority of implementation of social programs of society development. It will be promoted by creation of sufficient legislative base and modern organizational administrative approaches in realization of sanatorium services.
