Abstract. The dynamics of the prevalence of longevity in Kazan province and Tatarstan Republic has been analyzed in 1897 and 2013. Over this period, the population at the age of 60 and older had increased 4,5 times, while the number of men had increased 3,3 times, women — 5,6 times. Consenescence is mainly due to the female part of the population. During this period the number of men at the age of 90 and older had increased 1,5 times, women — 7,1. The group of centenarians is formed by the female part of the population. The prevalence of long-living persons per 1 000 of population had increased in 161% and 2,3‰. Longevity indicator, calculated by E. Rosset method, had increased from 1,23 to 1,26%. The prevalence of centenarians among men had decreased in 7,4%. 218 people at the age of 100 and older (5,70 persons per 100 thousand of population) had lived in Tatarstan Republic in 2013.

Abstract. The article presents the analysis’s data of demographic indicators of the citizens of Orenburg region who had done military service earlier and now reserved as potential candidates for entering the contract military service in Russian Armed Forces. The health assessment of future members of contract military service has been given. The level of physical development and physical proficiency necessary for the differentiated approach to raising troops of combined military forces has been determined. Results of the research showed that the age structure of this category of citizens was characterized by prevalence of persons at the age from 19 till 25 (72%), at the age from 26 till 30 (20,4%) and at the age from 31 till 40 (7,6%). The rate of citizens with medical classification «A» for the studied period had constituted between 86,4% (the maximum indicator) in 2005 and 74,1% (the minimum indicator) in 2008, and with medical classification «B» between 25,3% and 12,7% respectively. 99,4% of future contract members considered themselves as almost healthy. 80,8% of respondents seldom had catarrhal diseases. Results of physical proficiency were: only 82% of testing persons ran the 1000 meters’ distance with necessary result but 99,7% of citizens reached the standard for pull-up and 100 meters’ distance running.

The study’s results of physical development of service persons, doing call-up military service in Orenburg region

Abstract. The article presents the study’s results of physical development of 500 service persons, doing call-up military service in the Army in Orenburg region. In the first month of military service only 17,0% of service persons had normal (average) physical development, 52,6% — under average and 30,4% — low physical development. After 3 months of military service we found out the decrease in the proportion of low physical development up to 6,8%, and increase of number of service persons with average and under average physical development up to 32,4 and 60,8%. It indicates the improvement of physical development of service persons after 3 months of military service.

Abstract. Multistage system of forecasting of coronary pathology has been proposed on the basis of existing innovations in mathematical modeling of some diseases of the circulatory system. The constructed model of public health is dynamic, it takes into account common factors of change of the studying phenomena in certain period of time, patterns of change over time of influences of the risk factors of cardiovascular diseases’ emergence, delay of influence of risk factors of diseases of the circulatory system. The random-number generator has been used, formed unnatural uniform binary code G of length n with error multiplicity t, certain number of information bits k, in terms of Hamming distance dmin. The number of information bits k and power of unnatural code G has been determined. Generating matrix of the linear group code length n has been developed. Highlighted information and check part have been pointed out. Generated binary vectors (no more, than 20) of G-code have been recorded in text file line by line. dmin, corresponding polar radius point in the polar reference system, has been defined for each vector. We used the software implementation of the imaging algorithm based on the range of dyadic vectors using statistical data on the primary circulatory diseases incidence. A new type of metric space generated by series of spirals with closed form corresponding to the phase trajectory of the studying process is developing for any phase. The form deviation determining the image of human miosis and its representation as shape of lobe to the center, in the field of negative values, correspond to the reduction of morbidity and can serve as additional criteria for institutional control of quality and safety of medical care. Imaging analysis of miosis of CSD and its representation in the form of lobe’s shapes indicate the scenario of phasing out of all negative lobes, growing in positive direction.

Abstract. The purpose of this research was the study of medical-social aspects in acute drug intoxication. Multicentre medical-social study of acute drug intoxication has been conducted with the technology of factor analysis. This method of statistical studies allowed us to determine the leading causes and risk factors of acute drug intoxication at the population, which are necessary for optimization of organizational and managerial decisions during the preventive measures of this pathology.

The role of birth weight in infant mortality after the example of Omsk region in 2004—2013

Abstract. 34,5% decrease of infant mortality indicator has been noted in Omsk region in the period of 2004—2013, and was due to its 36,2% reduction in the neonatal period. The analysis of infant mortality among children of various birth weight categories has allowed identify a number of lows. The 5,5 times increase of this indicator among the children with extremely low birth weight has been noted during the analyzed period what can be explained by transition to new criteria of live birth in 2012. Positive dynamics in decrease of infant mortality was noted in all weight categories: 1000—1499 grams — for 36,0%, 1500—2499 grams — for 65,6%, more than 2500 grams — for 40,5%.

Abstract. The analysis of paid medical services providing in Tatarstan Republic in 2010—2013 showed the paradoxical feature of their volumes’ increase against annual increase in the government health care expenditures. Justification of objective factors, influencing the increase of paid medical services has been given. These factors include absence of accurate regulation of procedures for the provision of services at federal and regional levels, low level of branch financing, staff deficit, outdated standards of workloads for the personnel diagnostic examinations. Absence of tariffication on separate medical services within the finished case of treatment in hospital leads to frequent cases of double payment — at the expense of means of compulsory health insurance and personal means of the patient. Leveling of the factors, promoting the increase of paid medical services, will promote decrease of strife of the parties (one – paying and the other – providing medical care according to the compulsory health insurances programs) and also social discontent of patients.

Abstract. Necessity of development of legal, regulatory and methodological support of the sanitary-epidemiological well-being of participants and guests of international public event and Tatarstan Republic population, taking into account the risk analysis, has been proved. Legal-procedural framework of Federal Service on Customers’ Rights Protection and Human Well-being Surveillance, Tatarstan Republic Government and institutions to the emergency preparedness of sanitary-epidemiological nature in the period of Universiade 2013 has been analyzed. Updating of existing regulatory documents has been conducted, the necessity of development of new ones has been proved. These measures allowed increase the emergency preparedness of sanitary-epidemiological nature of healthcare and sanitary-epidemiological institutions, master the elements of effective interdepartmental interaction during the next stage of preparation the public event with international participation.

About the productivity of researchers’ training with specialization in «Public health and health care» and the problems of formation of evaluation criteria of research advisors’ activity (for discussing)

Abstract. The analysis of the research staff ‘s training in the sphere of public health and health care on example of the public health, economics and health management sub-faculty of Kazan State Medical Academy in the period of 1971—2014 is presented. 59 dissertations were defended from110 individually planned themes of scientific researches with specialization in public health and health care (41 in candidacy for scientific degree and 18 — for doctor of medical science) or 53,6±4,78 on 100 themes. The average age of candidates of sciences is 40,2 ±1,23 years old, of doctors of science is 47,7±1.3 years old. The ratio of applicants for degree is to the advantage of men: 1,4:1 (male/ female) among all, 1,2:1 among applicants for candidate degree, 2:1 among applicants for doctoral degree. Most prepared to achieve the final result were groups of 30—39 years old and 40—49 years old people, where more than 60% have defended their thesis. The 23—29 years old women presented the most inefficient group with the frequency of defended thesis 26,7 per 100 themes.

The importance of the aspects of the medical staff’s social structure for personnel management in multipurpose public health institution

Abstract. The work presents the research of medical staff’s structure according to age and gender in state multipurpose medical institution and aspects of social structure’s influence for its formation. The most important factors increasing the attractibility of health institutions as working place for medical staff have been identified. The research was based on the questionnaire survey of the physicians (174 persons) and the personnel department’s data of Volga District Medical Centre (VDMC) of Federal Medical and Biological Agency (FMBA). According to received data, the researched staff is characterized by practically parity relations between men and women and the proportional distribution of the specialists by age. At the same time, the leading motivational factor in the choice of the employer by the third part of the physicians was the opportunity for the professional development. Only 16,1±2,7% of the physicians were focusing on the salary. The following factors also had statistical importance: 17,0±2,9 % of the physicians gave the priority to the friendly relations between the employees, 10,1±2,3% underlined the opportunities for work with the modern equipment, 10,5±2,3% mentioned good working conditions. For 5,9±1,7% of the respondents the most significant factor turned out to be the caseload; the same was the proportion of the physicians , who indicated the importance of convenient schedule. The authors also conducted the analysis of the employment determinants for physicians, according to their age and gender. The desire to improve their professional skills is common for the specialists of all ages. However, the role of the given factor during workplace’s choosing decreases in the process of transition to elder age groups; and the importance of the salary, on the contrary, increases. For the specialists, who are older than 55 years old, the most important is the caseload and the convenient schedule. Furthermore, the salary, as the leading factor in determination of the attractiveness of the job, is more important for men than for women, who are largely guided by the convenient working hours. The study indicates a high level of professional migration of the physicians: for the last decade the researched medical staff has been renewed on 86,6±4,0%, and 60,6±5,8% of the professionals work at the given health-care institution for less than 5 years. At the same time the authors didn’t manage to denote the dependence of the frequency of the change on the physicians’ gender. According to the received information, the significant part of the physicians (41,0±3,7%) acquired other health profession during the first five years after the postgraduate education. As the correlation analysis shows, the gender characteristics do not determine frequency of employer’s or profession’s change. At the same time, the features of the social structure have an impact on the leading motivational factor during workplace’s choosing.

To the question of efficiency improving of medical institution’s activity

Abstract. Today the experts pay more attention to effective management taking into account the importance of the implementation of government guarantees to the population in medical care providing in the necessary level with high quality. In this regard, there is the need to evaluate the medical institution (MI) by its productivity and efficiency. Unfortunately, today there are challenges in definition of the efficiency. Classical Russian point of view suggests the need to identify medical, social and economic efficiency of medical institution. WHO calculates two integral criterions of healthcare system: 1) the general index of health system goals’ achievement (equivalent of medical and social efficiency) 2) the efficient use of resources to improve health (economic efficiency). The standard ISO 9001-2009 determined that two components can be the output of the process: efficiency — effectiveness of process — the ability to achieve the stated outcomes and effectiveness — efficiency of process — the achieved results, taking into account the expended resources. These concepts are different but at the same time, they are related to each other. Any efficiency contains high or low efficiency, depending on the basic element of comparison (performance standards, the industry average, the results of the previous period, a comparison with similar institutions, etc.). The authors think important to determine the concept “MI efficiency” and to discuss possible indexes and management tools to its increasing, which can be applied in practice of domestic medical institutions.

Abstract. The article presents the results of study of the staff’s views of licensing bodies and license competitors for medical activity on the further development and updating of the structure and function of licensing authorities. The functions’ reinforcement of the Commission on compliance with the General civil servants’ official conduct and the inclusion of representatives of civil society have been suggested as the improvement measures.

The dynamics of life quality indicators of military population

Abstract. The article presents the results of dynamic research of life quality indicators among 500 militaries, serving in Army in Orenburg region. International standardized russian questionnaire MOS SF-36 was used. The survey was conducted twice: on the first & on the fourth month of military service (in the end of adaptation period). We found out that the summary indicators of physical & psychological components of life quality on the 1-st month of the survey were 53,56 & 62,93 points; on the 4-th month they went up to 55,21 & 65,48 points. Thus, we statistically found out the improvement of life quality of military population since 3 months from the beginning of military service.
