Parkinson’s disease in general practitioner activity: clinical diagnostic app roaches

Abstract. Diagnosis of Parkinson’s disease (PD) is made clinically and its diagnostics can be difficult. On the basis of study

of PD incidence rate in one of the districts of Chelyabinsk an original questionnaire of the main symptoms of PD was worked out.

Due to the data of this questionnaire the number of revealed patients with PD increased by 10% approximately. The conclusion

was made that in carrying out differential diagnosis in elderly patients family physicians should be vigilant against PD. For the

first time the symptom «automatic watch» contributing to detecting hypokinesia was described.

Epidemiology of blood tumor diseases in children in the Udmurt Republic

Abstract. The article presents data on the sickness rate and prevalence of diseases of blood of tumoral nature in children

in the Udmurt Republic. A retrospective and prospective epidemiological study was conducted. The follow up period was 14

years — from 1996 to 2009. The average annual incidence of acute lymphoblastic and myeloid leukemia in children was 3,3±0,2

and 0,6±0,2 per 100 thousand children respectively. The sickness rate for children with non-Hodgkin’s lymphoma was 0,9±0,2

and Hodgkin’s lymphoma — 0,6±0,2 per 100 thousand population of the corresponding age.

Some epidemiological aspects of primary brain tumors incidence in Minsk population

Abstract. The results of epidemiologic analysis of statistical data on the incidence of primary brain tumors in Minsk population

are presented. The highest incidence of brain tumors among Minsk population is revealed in the age group of 60—74 years of age.

Among women the age groups of 60—64 and 65—69 years of age are the most disadvantaged regarding oncologic diseases

of the mentioned category, among men — of 65—69 and 70—74 years of age, among children — of 0—4 years of age for boys

and of 5—9 years of age for girls. These age groups are the groups of regular medical check-up and can serve as an epidemic

model for indentifying risk factors.

The situation in the sphere of labour migration and its medicо-demographic aspects in the Republic of Tatarstan

Abstract. Medico-demographic aspects of labour migration are shown on the example of Tatarstan Republic. The situation

on the detection of HIV infection, hepatitis, STD, and TB among legal migrants, compared with the resident population of the

Republic of Tatarstan is observed. The necessity of steps to be taken to improve medical examination and organize medical care

to migrant workers is justified.

Analysis of children’s mortality in the Republic of Tatarstan for the period of 2004—2009

Abstract. On the basis of the analysis of children’s mortality in the Tatarstan Republic for the period of 2004—2009 lowering

of this rate from 103,2 cases per 100 000 people of the corresponding age up to 70.3, or by 31,9% is noted. The leading causes

of mortality of children of 0—17 years of age are revealed: traumas, poisoning and some other consequences of outer causes

influence, certain states at the perinatal period, congenital anomalies. It is also noted that children’s mortality in rural regions

is higher than in urban ones (according to data for 2009, by 27,6%), and level of boys mortality is higher compared with girls

(according to data for 2009, by 44,3%).

Medico-social features of formation of digestive organs diseases in schoolchildren of Kazan

Abstract. The effect of 28 adverse factors on the emergence and development of digestive organs diseases among

schoolchildren of the 1st—11th classes in Kazan. Method of ANOVA evaluated their strength and hierarchy of their influence

2, %). Based on the data on prognostic coefficient a prognostic table is developed and thresholds of prognostic index are

calculated, which determine the probability of digestive organs diseases risk among schoolchildren. The data obtained allow us

to determine the need and volume of follow-up, develop actions to remove and reduce the adverse socio-hygienic and medicobiological

factors in a given student, and later on to develop preventive measures, taking into account the individual prognosis.

Problem of puerperal infectious-inflammatory diseases and the significance of its solution for improving state of mother’s health

Abstract. In the presented review the urgent problems of infectious-inflammatory diseases in puerperal period, significantly

influencing mother’s health are analysed. The leading role of puerperal endometritis (PE) is determined. It is stated that, subinvolution

of uterus is one of the most important predisposing factors of PE development. Characteristics of microbial spectrum of PE causative

agents are given. The role of conventionally-pathogenic microflora in the development of postpartum infection is revealed. Data

on the role of Campylobacter infection in the etiology of inflammatory diseases in endometrium are presented, its significance

among causes of pregnancy loss is defined and the necessity for studying its influence on postpartum complications is shown.

Occupational allergic morbidity in the Republic of Bashkortostan

Abstract. The literature data provide evidence that allergic pathology related to industrial exposure has become an urgent

problem in all developed countries. The authors highlight the elevated incidence of allergoses detected among workers of a variety

of occupational groups. In Bashkortostan Repulic also the analysis of prevalence of leading nosologic forms of occupational

morbidity of allergic etiology is conducted, in the research the data from statistic registering cards and registers of firstly identified

occupational diseases in Ufa Scientific Research Institute of Occupational Health and Human Ecology between 2000 and 2009 has

been used. It is determined that annually 150 to 200 new cases of detected occupational morbidity are registered. During the study

period, 1426 occupational diseases were firstly identified. The incidence of allergic pathology in the general structure of allergic

morbidity has been from 7,3% in 2008 up to 23,66% in 2001. Occupational diseases of allergic genesis are the most common in

individuals with length of work more than 11 years. Females (68%) are predominantly affected by allergic pathology. The leading

nosologic forms are occupational bronchial asthma (47%), eczema (22%) and allergic dermatitis (10%). Health care workers are

subjected to occupational allergic diseases most. They are followed by agroindustrial complex and construction workers.

The results of additional prophylactic medical examination of working rural population in 2008—2010 (on materials of Mouslyumovo district of the Republic of Tatarstan)

Abstract. In Mouslyumovo district 1 401 working rural citizens underwent additional prophylactic medical examination in

2008—2010. In 427 of them (30,5%) the diseases and anomalies were detected for the first time. As the data analysis has shown,

anomalies detected in instrumental examination of mammary gland take the leading place — 16,2%, visual impairment — the 2nd

place, and diseases of circulatory system — the 3rd place — 12,6%. All patients of this category underwent additional diagnostic

examination including republican centres as well. According to the additional examination results 281 patients are registered in

a dispensary with a confirmed diagnosis and take planned treatment. Average cost of examination of one citizen was 1 173,73

roubles, cost of detecting a disease at a preclinical stage and at a low symptomatic course stage was 5 851,95 roubles.

Medical and economical aspects of forming the health capital of children up to 6 years old with perinatal injury of central nervous system

Abstract. In the article the substitution of such a new concept as «the health capital of the individual» and its definition are

given from the position of public health and public health economy. Besides, possibility of defining the cost of the individual’s

health capital with the help of formalized values of health potential, as well as state investments and family expenses is proved.

A concrete example of calculating the health capital cost of the child first 6 years of life with perinatal injury of central nervous

system is presented. As shown, within 6 years the child kept his health potential due to his family expenses, which 20 times have

exceeded the state investments.

Economic aspects of surgical and endovascular treatment

of patients with multi-focal atherosclerosis in settings of a high-tech medical hospital

Abstract. Assessment of the cost-effectiveness of different methods of revascularization (surgical and endovascular) in patients

with combined atherosclerotic involvements of coronary, carotid, renal arteries and arteries of lower extremities is carried out in

this article. Such analysis methods as cost minimization, clinical and economical modeling were used for economic calculation.

Sufficient differences in the cost of surgical and interventional procedures while revascularization in patients with combined

atherosclerotic diseases are revealed. The use of simultaneous endovascular interventions can significantly reduce not only

the in-hospital treatment period, but also the patient’s rehabilitation period, thus intensifying the medical process, increase the

economic, medical and social efficiency of the treatment.

Some peculiarities of organization of intensive care service

(according to the data of the Republic of Tatarstan)

Abstract. This article presents results of scientific research performed in resuscitation and intensive care departments in

different clinics of the Republic of Tatarstan and on their basis the social-hygienic, organizational and economic factors, influencing

the work of experts in resuscitation, are found. While correlation and dispersal analysis the reliably significant factors, influencing

the patient’s hospital stay time in the resuscitation department, are revealed. The prognostic table of the patient’s hospital stay

time in the resuscitation department is developed. With account of the cost of the one-day stay of the resuscitation patient, of

the hospital’s profile and expected time of his stay and on the basis of the presented table the financial expenses for patient’s

treatment in resuscitation department can be calculated and financial expenses for medical aid can be efficiently planned.

Social status of patients suff ering FROM benign prostatic hyperplasia

Abstract. The article presents the results of social-hygienic study of men with benign prostatic hyperplasia. More than half

of investigated contingent are elderlies (60—74 years old), every fifth patient — either able-bodied (60 years old) or old (75—

90 years old) man and less than 3% are long-livers (aged over 90 years old). Most respondents are old-aged pensioners, 15% —

disability pensioners, and about a quarter of patients are working citizens who are either able-bodied, or continue working after

reaching pension age. A minor part consists of patients who have reported other social status. Special attention in our study is

paid to financial position of patients, because it greatly affects the choice of means and methods of treatment. The vast majority

of patients either totally unable to spend money to buy drugs for the treatment or agree to spend from 300 to 1 000 roubles

per month, which cannot provide long-term use of modern and the most effective drugs. Disabled people (about 15%), having

benign prostatic hyperplasia as the main disease, now can use rather expensive medicines for treatment without compromising

their budget.

Organizational-technical aspects of treatment of prostate gland adenoma

Abstract. Analysis results of usage of surgical methods in the treatment of patients with prostate gland adenoma in the

settings of two large urological departments of the Republic of Tatarstan [the Republican Clinical Hospital (Kazan) and Central

Regional Hospital (Bougoulma)] are presented in the article. The aim of the research was improvement of techniques of open

operations with account of the size and location of adenomous nodes, successfulness of surgical methods of treatment. The

result of the study was development and introduction of retropubic extracystic urethrosaving prostatectomy into work practice.

47 patients were operated using this method. The advantage of this method lies in the fact that the operation is carried out in

 

more accordance with anatomy and atraumatically, and no dysuria, urinary incontinence and non-keeping are observed in thepost-operational period and this reduces the time of the patient’s hospital stay. All the above said increases the life quality of the

patients of this category.

Prevalence of latent forms of thyroid gland pathology among the population

of Tatarstan Republic (on the basis of screening laboratory researches)

Abstract. The results of screening researches of the latent thyroid gland pathology prevalence, structure and frequency

among working population of Tatarstan Republic are presented. The screening researches included the analysis of thyrotropic

hormone level (TTH) and level of antibodies to thyroperoxidase (Ab to TPO), using method ALISA on kits of «Hema» (Moscow)

and «Alcor-Bio» (Saint Petersburg). Among 1 766 examined patients 133 cases of hightened level of Ab to TPO and 55 cases of

dysfunction of thyroid gland activity were revealed. Frequency of the pathology was different among men and women, increased

with age. The data informed on prevalence of thyroid gland pathology among Tatarstan Republic population and on necessity to

increase screening medical examinations with the aim to expose the pathology at early stages.

The polyclinical model of an outpatient treatment centre: formation stage

Abstract. The only real way of solving the problem of rational use of a rising in price in-patient bed is expansion of out-hospital

stationary-replacing aid. One of such trends is the one-day surgery (outpatient surgery). Among the forms of its organization the

most difficulties arise while creating its polyclinic model. The optimal variant of its organization and rational activity during the

formation stage is testified on the example of successful work of such a model. The authors’ work experience bases on creation

of the centre, where more than 6,5 thousand patients went through treatment. During treatment over 60% of them had usual

in-hospital surgical interventions.

Possible directions of efficient functioning of the Emergency Service

in the Republic of Tatarstan

Clinical effectiveness of medical-social prophylaxis of concomitant chronic dermatosis in patients with psychic disorders

Abstract. Medico-social prophylaxis of concomitant chronic dermatosis in patients with psychic disorders with account of

influence of unfavorable risk factors of beginning and their clinical course is performed. Clinical effectiveness of prophylaxis is

studied, practical measures on diseases prophylaxis in the settings of a dispensary are worked out.

The state of free radical oxidation of the oral liquid in petrochemical workers

Abstract. While studying free radical oxidation (FRO) of the oral liquid of 2 000 petrochemical workers using the method of

chemoluminescence (CL) we could observe nonspecific protective response of the organism exposed to xenobiotics. Normal CL

indicators of the oral liquid were determined. The CL studies of the oral liquid in workers with dental disorders revealed significant

changes in the indicators. These indicators did not depend on length of work under hazardous conditions. They depended on

severity of dental disorders alone. In persons with diseases of the parodontal tissues and oral mucosa, CL indicators of the oral

liquid were not homogeneous. There was an increase in oral liquid CL in 1 402 workers and a decrease — in 383 workers. The

data obtained confirm the necessity to consider the state of free radical oxidation of the oral liquid while choosing treatment and

preventive measures.
