The state and the character of changes of primary disability rate due to cardio-vascular incidence in Sverdlovsk oblast

Abstract. The results of changes’ evaluation in primary disability rate (PDR) caused by the diseases of blood circulation system among adults of Sverdlovsk oblast are given in the article. Significant decrease of PDR level (on 36,6%) was noted during the period from 2007 to 2011, including 44,2% of PDR caused by ischemic heart disease (IHD) and 27,5% of PDR caused by cerebra-vascular diseases (CVD). Intermunicipal difference of these parameters according to rate value has been defined. We had made the conclusion that the main cause of significant decrease of PDR level due to IHD and CVD in Sverdlovsk oblast was closely connected with the implementation of Federal program measures dedicated to the improving of the system of medical services’ providing to patients with vascular diseases.

Abstract. The analysis data of hospitalized patients with stroke incidence on example of primary vascular unit of SBHI «Municipal Clinical Hospital № 5» are presented. New procedure of medical care rendering has been examined. Organization of vascular centers in Russia has improved the quality of medical care rendering for patients with stroke and systemic thrombolytic therapy has significantly reduced the incidence of early (hospital) mortality and disability of patients.

The main tendency of HIV epidemic and the dynamics of incidence of sexually transmitted infections

Abstract. The results of comparative epidemiological analysis of STI and HIV infection for the period from1997 till 2012 on the example of Magnitogorsk in Chelyabinsk region are given. Influence of STI on the main trends of HIV infection’ epidemic development, including the development on the base of study of the correlation dependence between HIV and STI incidence in total, HIV and STI incidence in women, HIV infection and diseases which constitute the structure of STIs, HIV and syphilis has been shown. The study showed the tendency of the registration’s increasing of new cases of STI and HIV infection among women, the inverse correlation between the incidence of HIV and syphilis, which means the reducing of risk of HIV through heterosexual mechanism when the number of persons, suffering from sexually transmitted diseases, for specialized medical care is growing. Monitoring of infections with common mechanism of expansion enables us to justify the complex strategy to prevent HIV infection.

Abstract. The goal of the work was to evaluate the dynamics of health state of students entering the highest military educational institutions, depending on changes in social and economic situation during the period of formation and development. 18-years old students have been distributed in groups taking into account the different level of average annual gross domestic product’s size in Azerbaijan (during 18 years of their life). Connection between indicators of health and social-economic situations of formation and development of the personality on the example of students has been established.

Birth rate’s parameters necessary to overcome the level of simple population’s reproduction and the startof its growing (on example of Tatarstan Republic)

Abstract. The choice of the most economical way of the definition of the limits of the simple population’s reproduction is possible with different parameters of birth rate (the intensiveness of procreation, the living out of daughter generation and the level of general birth rate). Methodology of the soviet demographer B.S. Yastremsky, illustrated by calculations for Republic Tatarstan has been used for commentary of connection between parameters of population’s reproduction. The analysis of dependence of given parameters has been done. Designing of necessary level of general birth rate coefficient, with providing the simple replacement of parental generation by generation of their children has been realized. The study’s results can serve the argument for state programs’ creation directed to the demographic development or can be the instrument to their correction in the course of realization.

Abstract. The work presents the dynamics of the main demographic and health indicators of the population of Moscow for the period of 2005—2012, obtained in the course of complex social-hygienic research. General morbidity of the population of working age increased on 4,9%, primary one – on 6,5%. The first place in the structure of mortality causes has been given to external ones (34,6%), the second —to diseases of the circulatory system (25,3%), the third – to neoplasm (12,7%). During the designing of the trends, it has been found that the level of mortality rate has tendency to increase — R2=0,422, despite of reduction of mortality rate in 2012. Thus, the complex social-hygienic study has revealed a number of features which affect the health state of the population and the system of organization of medical service.

Long-term effects of nursing of extremely low birth weightinfants

Abstract. The goal of the study was the determination of formation rate of severe neurological diseases in extremely low birth weight (ELBW) infants. The observed infants have been divided in 3 groups: first group (68 infants) – with ELBW, second group (87 infants) – with low birth weights (1 001—1 500 g), third group (50 infants) – with birth weights 1 501—2 500 g. In long-term period in the first group – the rate of cerebral palsy was 77,9%, epilepsy 11,8%. Disability rate was 39,7%. In the conclusions we can say that severe neurological disorders are typical for ELBW infants. The proposed monitoring algorithm makes possible to optimize the rehabilitation measures.

Abstract. The goal of the work is to analyze the results of the use of existential-humanistic method «preventive psychodrama» and personality-oriented training programs on prevention of psychoactive drug abuse (PAD). Observation in the form of anonymous questionnaire has been done at the beginning and at the end of the school year in two groups of teenagers of eighth-tenth forms. Statistically significant signs of increasing motivation of the refusal from PAD have been revealed in the experimental group of teenagers. This innovative method can be used for PAD prevention in teenagers.

Abstract. Decrease of financing size due to world financial crisis makes negative influence on development process of dental institutions. Though increased competition, scientific and technological progress, the state support of innovations let us claim that there are preconditions to stimulate the activity of dental institutions. Economic potential analyze of AHCI «Municipal dental policlinic» within 2008—2012, its economic efficiency, has been done. Interrelation of different indicators of economic activity included into book-keeping reports has been revealed. Postponement of given services has been introduced. The consolidation strategy had led to improvement of financial state of the institution. The introduced policy of enlargement led to the improved financing strategy. As the result the institution hasn’t overdue accounts payable, credit debt is frequently used as one of the sources of the institution assets formation.

The role of complementary medicine techniques in the optimization of preventive and curative care to children with stammering

Abstract. The authors have conducted complex clinical electro-neurophysiological examination of 39 patients with stammering in age from 5 to 15 years old, which included detailed study of the case history, clinical neurological, osteopathic, psychological research, the study of the functional state of corticospinal tract by magnetic stimulation, polysynaptic component of the blink reflex, monosynaptic H-reflex. As control, 20 children comparable by age and sex with the main group have been examined. Psychological testing of parents has been conducted at the same time. Results of clinical studies indicate the presence of various degrees of severity of minimal brain dysfunction, which indicates non-robust organic changes in central nervous system and proves transcranial magnetic stimulation in the course of which slowing of spread of excitation in corticospinal tract was revealed. The monosynaptic H-reflex various options of reflex responses have been identified during studying of the late components of the blink reflex. Results of the study suggest about violation of the dynamic interaction of segmental and suprasegmental structures on the background of deficiency of descending inhibitory effects leading to increased reflex activity predominantly of oral sections of the brain stem and segmental apparatus of the spinal cord in children with stammering. Differentiated therapy, based on the studies’ data and using the methods of traditional medicine, directed on realization of the basic mechanisms of speech disorders, which contributed to the favorable clinical outcome of the disease, helped to prevent its recurrence, has been applied.

Epidemiological and clinical aspects of humeroscapular pain syndrome

Abstract: The article presents the clinical and epidemiological data characterizing humeroscapular pain syndrome. Pain in the shoulder joint area is one of the most common complaints connected with musculoskeletal system among the adult population. The questions of etiology, pathogenesis of main clinical variants of humeroscapular pain syndrome have been examined. The modern treatment methods have been presented briefly.

Abstract. The histologic structure of incidence of benign diseases of thyroid gland taking into account biogeochemical division into districts has been studied for the first time in Chuvash Republic. The study had revealed the increased incidence of autoimmune diseases in Prisursky subregion, high percent of diffusion-toxic goiter and nodal-toxic goiter with formation of functional autonomy in Privolzhsky subregion. Prikubninotsivilsky subregion has been separated as the control one during studies of endemic goiter in Chuvashia.

Abstract. The article presents the organization of medical maintenance of Universiade 2013 in Kazan. The main health statistics have been given. 12 940 medical consultations of Universiade client groups had registered in total. First ranking places of the use of health care has been given to the athletes — 383,7‰, NSSF officials — 328,9‰ and the judges — 221,7‰. The highest prevalence of injuries among athletes is typical for the following sports: belt wrestling, sambo, gymnastics, wrestling, judo. Leading positions in the structure of injury have been given to the following parts of the body: hand, wrist (18,4%), knee (16,2%), foot (11,7%), head, face (10,8%). The presented data have the practical significance in the conditions of preparation for the forthcoming international sports competitions.

Abstract. The article presents the information of social and hygienic situation’s analysis in connection with the treatment of Russian citizens abroad. The goal of the study was to identify motivational factors that encouraged Russian citizens residing in the territory of Primorsky krai to address to foreign clinics for medical services. These are the results of the study: the patients of oncological profile (22,7%) take the leader place; one of the main reason for the treatment in the foreign clinics was to get examined and treated in high modern level; the second place was given to inadequate equipment of medical institutions, and as result the lower level of diagnostics; the third place was given to dissatisfaction by the qualifications of medical personnel and the desire to get higher health care quality.

Abstract. The goal of the research was to develop the methods of maintenance and restoration of independence of the patient in satisfaction of basic needs of organism. Questions of organization of nurse process during treatment of patients in post-stroke period have been studied with using of Bartel index estimating tables, Waterlow scale estimating sheets (risk of decubituses’ emergence) on the example of neurologic unit. For improvement of nurse care quality to patients with stroke it is necessary: 1) to complete nurses’ personnel and paramedical personnel; 2) to train nurses to help patients’ relatives to take care of cardiac; 3) to train nurses on communication’s psychology. The study’s results can be used in the system of health care organization and neurologic hospitals.
