Abstract. Nowadays Kazan State Medical Academy is a coordinator of post-graduate education in the Privolzhsky Federal District, one of the biggest centers of post-graduate training of physicians in Russian Federation, former Soviet republics and far foreign countries. 2010 is an anniversary year for this educational institution — the academy is 90 years old. The data on achievements of KSMA, perspectives of its development are presented in the article.

History of the sub-faculty of public health, economics and public health service administration of the Kazan St ate Medical Academy: personalities and historical parallels

Abstract. Stages of formation and development of one of the leading sub-faculties of Kazan State Medical Academy — faculties of public health, economics and public health service administration which plays an important role in the system of postgraduate education of the health service executives of Russia (1926—2010) are presented in the article. The earlier unpublished manuscripts from the collections of the National Archive of the Republic of Tatarstan and their own archives were the materials for studies. The role of professors M.A. Dykhno, T.D. Epshtein, B.S. Ginzburg, E.M. Barkman and others in the sub-faculty history and its achievements is shown.

Demographic indices analysis in Azerbaijan Republic

Abstract. Revealing and researching of main tendencies and rules of demographic and population health indices dynamics create necessary prerequisites in forming scientifically grounded suggestions for improving national public healthcare system as a basic factor of public health protection. With that aim the main medical-demographic indices of public health in Azerbaijan Republic were considered in dynamics for the period of time from 2003 to 2007 including the comparison with published indices of several foreign countries. It was determined that annually the level of the new-born was much higher than level of the dead, the increase is from 1,3 times in 2003 (64 466 persons) to 1,8 times in 2006—2007 year (almost 100 000 persons) which was higher than in Russia and European countries. Average annual number has the constant positive dynamics due to the birth rate, which during the last three years has obtained a tendency to stabilization. Thus in the conditions of negative migration balance (population outflow) state of health of citizens of Azerbaijan Republic becomes a basic factor of demographic national security. In the light of the studied dynamics of cases of population morbidity subsequent investigations of demographic indices of morbidity, organization of medical and pharmaceutical aid as well as analysis of drugs use is possible, that allows to develop ways of

providing the population with medicines.

Modern aspects of management of antenatal prevention process

Abstract. Intensity of activity of functional systems of a pregnant woman corresponds to the extent of gravity of placental insufficiency. Accordingly, criteria of activity of functional systems display both the presence and the extent of severity of placental insufficiency. As a criterion of a functional system activity of regulation of microcirculation volume in gestoses and placental insufficiency one may use cardio-vascular index that represents the result of a ratio of average effective blood pressure to the frequency of contractions.

Medico-social characteristics of call-up age youth (on the example Orenburg region)

Abstract. The article gives the results of anonymous questioning among 1 500 youth of call-up age, living in Orenburg region. We made up their social portrait considering the level of education, profession, family status, economic and living conditions. As a result of questioning the prevalence of harmful habits among the respondents was determined. It was noted that more than 66% of the interrogated young men had harmful habits: among them 64% smoke, 74% drink beer, about 50% drink spirits.

Basic indices of Am ur region population public health

Abstract. Basic health indices of population, such as mortality rate, are presented; this index increase is shown from 1990 to 2007 by 1,7 times. The causes of mortality are reflected: the first place occupy cardio-vascular diseases, the second — accidents, traumas and intoxication cases, the third — tumours. Data on infant and maternal mortality are presented. Indices of disease incidence in adults and children are considered, as well as indices of socially important diseases (tuberculosis, syphilis, malignant tumours). The problem of disability is depicted. Means of public health improvement are revealed.

Health state of pre-school children in social risk group families

Аbstract. With the purpose of development of practical recommendations on organization of work with children from unfavourable families the socially-hygienic characteristics of families of risk group families are drawn up, the estimation of physical, neuro-psychic development and morbidity of children from unfavourable families is carried out. Sociological, clinical and analytical methods are used in work process. The results of investigation have shown significant developmental lagging and high rate morbidity in children (mostly on socially important and psycho-somatic diseases).

Monitoring health state of school children

Abstract. The man's health begins to form particular at school age. Study results of health state of school children in 90-s and nowadays are analyzed in a comparative aspect. The clinical evaluation of health state was studied according to the method developed by the Scientific — Research Institute of children’s and adolescents’ hygiene. Impairment of their health state was revealed, as well as decrease of number of healthy children and increase of number of children with functional disorders and

chronic pathology.

Complex US using Doppler technique as means of gestosis diagnostics quality improvement and prognosis of its course

Abstract. This work is based on examination results of 103 women: 78 pregnant women with different degree severity gestosis and 25 patients from gestosis development risk group. Examination of the pregnant women included traditional clinical laboratory diagnostics, complex US using Doppler techniques of blood flow in great vessels of liver and kidney. Peak systolic blood velocity (Vmax), resistance index (RI) and systolic-diastolic ratio (SDR) were defined. In the result of research the peculiarities of hemodynamic parameters in renal and hepatic vessels in pregnant women with gestosis and in the risk group of its development were identified. And this makes possible to prognoze the disease course and to optimize its treatment.

Peculiarities of long-term dynamics of occupational morbidity

Prognostic value of risk factors in development of cancer

in reproductive system organs in women

Abstract. With the aim of definition of prognostic value of risk factors in the development of cancer in the reproductive system organs in women of fertile age, the retrospective epidemiological analysis of morbidity was carried out in the Republic of Tatarstan in 2007—2008 (10 393 questionnaires) on the base of polling by questionnaires and personified data of the cancerregister of the Republic of Tatarstan. High relative risk of cancer development was revealed in respondents of questionnaires (exceeds the population risk by 17,5—72,2 times). After grouping and classification of risk factors the authentic communication was established between the presence in women of malignant tumors and unfavourable background conditions: obesity, stress, gynecologic diseases and traumas of a mammary gland in the anamnesis, family predisposition to cancer (Kramer’s criterion

was 1,154—1,624).

Efficiency of treatm ent of nephrologic profile patients in day hospital

Abstract. A comparative assessment of treatment results of 114 nephrologic profile patients in day hospital and in 24-hours nephrologic care unit, is presented, that served the purposes of introduction of optimally effective methods of treatment for rational use of specialized nephrologic care unit beds. Sociological, statistical and analytical methods were used in the work. Its results showed the medical, social and economic efficiency of the nephrologic patients in day hospital settings.

Characteristics of stomatologic health of children with congenital cleft lip and palate in Udmurt Republic

Abstract. The data of stomatologic examination of children with congenital cleft lip and palate living in Udmurt Republic is presented. The need and requirement for treatment at children’s stomatologist-therapeutist, stomatologist-orthodontist is determined. The level of stomatologic care rendered to children of different age groups — poor for children under 6, insufficient for children of 7—12 years old, satisfactory for children over 12 years old. Research data can be used in planning and organizing stomatologic care for children with the above-mentioned defects.

Schizophrenia: selfstigmatization, quality of life and problems of patients’ rehabilitation

Abstract. This article is devoted to studying of quality of life, social functioning and selfstigmatization types of patients with schizophrenia. With the help of questionnaire of WHO “Quality of life” and stigmatization questionnaire of Psychic Health Scientific Center of Russian Academy of Medical Sciences, 345 patients suffering from schizophrenia were examined. Averaged profile of quality of life of patients with schizophrenia was developed. Three types of patient’s selfstigmatization were described. Correlative relations between indexes of quality of life and selfstigmatization were analyzed. Obtained data are necessary for developing individual destigmatization programs with the aim to improve patient’s quality of life.

Biopsychosocial characteristics of self-health-improving activity of adolescents

with psychosomatic disorders

Abstract. Biological, social and psychological characteristics of 200 adolescents suffering from psychosomatic disorders were studied. The main group included those who engaged in their health-improving activity on their own (121 adolescent, or 60,5%), the control group included adolescents not engaged in self-health-improving activity (79 persons, or 39,5%). Factors were determined promoting adolescents’ adherence to self-improving their health: older age, females, period of exacerbation, parents’ low educational level, poverty, incomplete family, uncomfortable living conditions, mild anxiety, ergopathic variant of attitude to the disease, selecting adaptive strategies of self-control. Self-health-improving activity of adolescents gave them the possibility of harmonizing their needs’ biopsychosocial structure. Promoted the disease reduction and improvement of socialpsychological

adaptation.

Dynamics of prostate benign hyperplasia incidence among the male population

of big industrial town

Abstract. Statistical data are presented characterizing prostate benign hyperplasia incidence, dynamics of its prevalence among males in Naberezhnye Chelny for the period of 2001—2007 taking into account their age. The data analysis has allowed to reveal quality peculiarities of the studied phenomenon dynamics, and to show that prostate benign hyperplasia incidence is increasing, more and more younger age groups of male population are involved. This is indicative of the necessity of strengthening measures on the part of health service institutions, first of all general medical network, on the early disease detection.

Organization and implementation modern x-ray technologies

for stomach cancer diagnostics algorithm

Estimation of rational use of budgetary funds by conducting ABC /VEN -analysis within the program of additional medicines provision

Abstract. The results of ABC/VEN-analysis of the FFOMI data on administration of medicines within the program of additional medicines provision for 2006 are presented. Analysis results indicate the irrationality of the spent budget funds. Ways of optimization of the implementation of programs of additional medicines provision, allowing lowering expenses and improving the quality of medicinal aid to the population are suggested.

Use of integral indicators for evaluation of intra-institutional efficiency

of central regional hospitals’ management

Abstract. While evaluating the efficiency of patients care institutions’ (PCI) management, a set of adequate criteria reflecting basic functions of the structure, processes and results of administrative activity is necessary. The aim of the study was usage of estimation principles of PCI management intra-institutional efficiency integral indicator on the example of two central regional hospitals of Sverdlovsk region. It was suggested to undertake a standardized approach to evaluation of public health institutions’

efficiency using the original formula for calculating the integral indicator with the help of qualitative and quantitative appraisal of separate indicators. The method made it possible to evaluate the intra-institutional management efficiency in concrete PCIs. Usage of the generalized integral indicator would help PCI managers and representatives of public health administration to assess objectively the level of intra-institutional management efficiency.

The indexes of mental and emotional state and quality of life

of patients with chronic obstructive pulmonary disease

Abstract. Mental and emotional state and quality of life (QL) of 83 patients with chronic obstructive pulmonary disease (COPD) were studied. The indexes of mental and emotional state (anxiety, depression) were studied with the help of the Zung scale and were compared with mental and emotional state of practically healthy 80 people (group of people, comparable by their age and gender with the patients with COPD). QL of patients with COPD was studied with the help of the special questionnaire SF-36, data of the research were compared with population standard, determined according to the results of study of 213 people QL. It was shown that the level of anxiety and depression among patients with COPD was obviously higher than in the group of comparison. The indexes of QL among patients with COPD were worse in all parameters comparing with the second group. The biggest decrease was registered on the scales of physical activity.

Quality of life of veterans of antiterrorist action in Chechen Republic

Abstract. It is presented the research results of quality of life (QL) of 412 participants of the Chechen armed conflict (CAC) with the subsequent estimation of dynamics of QL level during inpatient treatment with the use of Russian analogue of universal questionnaire «SF-36 Health status survey». It is identified, that both the common level and parameters of QL over all scales at participants of armed conflict are authentically lower, than at practically healthy group of comparison. The greatest distinctions are revealed on the scales: «Role emotional functioning», «Common level of health», «Role physical functioning» and «Social functioning», that is connected with high frequency of development of posttraumatic syndrome at the given category of the population. During long regular treatment (4 and more times) at veterans both integrated parameter of QL and its levels over all scales have authentically improved. The research results allow recommending the use of QL dynamics as criterion for estimation of treatment-and-prophylactic and rehabilitation measures efficiency, alongside with traditional indicators.

St udying the life quality of patients with obliterating endarteritis of lower extremities

Abstract. The aim of this work is studying the life quality of patients undergoing treatment with autologous stem cells of peripheral blood in the early stages of trials. During the investigation the following tasks were set: 1) to assess the life quality of patients with injury of lower extremities' vessels; 2) to determine the safety and efficiency of stem cells use in the treatment of patients with the pathology; 3) to assess patients life quality dynamics in the process of innovative treatment. In half a year in patients after stem cell therapy there was noted some increase of indices by all the scales of the WHO questionnaire «QL-100». Due to small quantity of participants in the investigation we cannot say about the statistical reliability of the results, nonetheless we can make a conclusion of a positive tendency.
